Abstract: Chronic Heart Failure is the outcome of all kinds of heart diseases. As the incidence of cardiovascular diseases increases, the number of existing and potential patients with Chronic Heart Failure will increase accordingly. Moreover, Chronic Heart Failure is prone to occur recurrently. Transitional care, as an important means to control the recurrence and deterioration of the disease, is of great significance to the rehabilitation of patients. This article reviewed the patterns of transitional care for patients with Chronic Heart Failure, in order to provide basis for future research.
