This article introduces an evidence-based intervention program–Tailored Activity Program (TAP), which was initaitally implemented in the United States to effectively alleviate the neuropsychological behavioral symptoms of older adults with dementia and reduce the burden on caregivers. It also discusses the challenges and strategies of localization, and provides feasible suggestions for the development of evidence-based intervention studies for the care of older adults with dementia in China.

This study introduced the concept of Dementia Friendly Community (DFC), its underlying principles, goals, activities and outcomes. Drawing from the framework of Alzheimer's Disease International and the Alzheimer's Society of the UK, this study proposed the 4P-EASTT model to delineate the core features of DFC. Guided by this framework, case studies of DFCs in Shanghai, Nanjing, and Battle Creek in Michigan of the U.S. were presented. Recommendations for practice and policy are made regarding the promotion of DFCs for interested agencies and communities in China.

Objective: To investigate the current status and influencing factors of nurses' hospice care self-efficacy. Methods: A total of 1052 nurses from 30 provinces and cities in China were recruited by convenience sampling method. The survey tools are a self-designed general information questionnaire and the nurses' hospice care self-efficacy scale. Results: The mean score of the total scale and each item of nurses' hospice cares self-efficacy was (70.55±10.55) and (3.92±0.59) , respectively. The dimensions scores from high to low were body care behaviors (4.02±0.63), family care behaviors (3.90±0.66), and psychological and mental care behaviors (3.85±0.65). Multiple regression analysis showed that working years, the past behavioral experience of end-of-life care and perceived importance were the main influencing factors of nurses' hospice care self-efficacy. Conclusion: The level of nurses' hospice care self-efficacy is in the moderate and high level, but the psychological and mental care ability needs to be strengthened. In order to improve nurses' hospice care quality, nursing managers should pay more attention to hospice care training and education, improve the nurses' recognition of hospice care, help to establish a correct view of life and death, strengthen the nurses' psychological care ability for patients and their families, and increase the accumulation of nurses' end-of-life care experience.

Objective: To investigate the effect of preventive interventions of perinatal depression on home nurture environment and the neurological and psychological development of infants. Methods: Totally 148 mothers with mild perinatal depression were randomly divided into two groups. Those in the experimental group were with cognitive behavioral therapy and mother-to-child relationship therapy in late pregnancy and the first postpartum 42 days. Those in the control group were with routine health counselling and mental health guidance in midwifery clinic during the same period. Results: The total scores and the scores of each dimension of Child Home Nurture Environment Scale, Child Nerve Mental Development Scale and Infant Social Response Questionnaire in the experimental group were better than those in the control group at the age of 3 months and 6 months (P<0.01). Conclusion: This cognitive behavior and mother-infant relationship therapy are beneficial for better home nurture environment and development of the neurological, psychological and social adaptive behavior of infants.

Analysis on body image disorder in patients with head and neck cancer and its influencing factors

Objective: To investigate the current status of body image in patients with head and neck cancer, and to analyze the influencing factors. Methods: A total of 106 cancer patients receiving surgery were surveyed by the General Information Scale, Body Image Scale (BIS), Perceived Social Support Scale (PSSS), Simplified Coping Style Questionnaire (SCSQ), and Hospital Anxiety and Depression Scale (HADS). Results: The total body image of patients with head and neck cancer was 5.00 (4.00, 7.00). Multiple regression analysis showed that anxiety emotion, disease stage, positive coping style, educational level and gender were the influencing factors of the patient's body image (R2=0.346, F=12.119, P<0.001). Conclusion: Patients with head and neck cancer experienced body image disturbance. Medical staff should pay attention to the patient's body image and formulate corresponding intervention measures to improve the body image level of patients and promote their physical and mental health.

The risk factors of Deep Venous Thrombosis in brain hemorrhage patients: a systematic review

Objective: To have a systematic review on the risk factors of Deep Venous Thrombosis (DVT) in brain hemorrhage patients. Methods: We systematically reviewed all articles related to the DVT risk factors in brain hemorrhage patients including Pubmed, Web of Science, the Cochrane Library, EMBASE, CNKI, Wanfang Data and CBM. Meta-analysis was conducted for quantitative analysis using RevMan 5.3 and descriptive analysis was adpoted as a supplement. Results: A total of 358 papers were retrieved, and 10 papers were included in the Meta analysis involving 19,447 patients with cerebral hemorrhage, of which 1,104 were diagnosed with DVT. Meta analysis showed that obesity [OR=9.45, 95% CI (1.81, 49.40), P=0.008], age [OR=3.84, 95%CI (2.57, 5.75), P<0.001], atrial fibrillation [OR=3.77, 95% CI (2.83, 5.02), P<0.001], paralysis [OR=6.73, 95% CI (1.69, 26.84), P=0.007], D-dimer [OR=607.42, 95%CI(488.19, 726.64), P<0.001]/[OR=8.24, 95%CI (6.36, 10.67), P<0.001], CRP values [OR=8.43, 95% CI (4.85, 14.65), P<0.001] and the average hospitalization time [OR=13.44, 95% CI (11.97, 14.90), P<0.001] were risk factors for DVT in patients with brain hemorrhage. The degree of neurological deficit assessed by National Institutes of Health Stroke Scale (NIHSS) was more applicable for western countries [OR=5.14, 95% CI (3.32, 6.97), P<0.001], but not for Asian countries [OR=9.06, 95% CI (0.58, 142.66, P=0.12]. Conclusion: Age, obesity, paralysis, atrial fibrillation, longer hospital stay, D-dimer and CRP were risk factors of DVT in patients with intracerebral hemorrhage, which could increase the incidence of DVT among those patients.

Objective: To explore the influencing factors of paternal breastfeeding attitude and to provide targeted suggestions for attitude improvement. Methods: From April to September 2018, the spouses of the pregnant women who attended courses in the prenatal education center of a public obstetrics and gynecology hospital in Hangzhou was investigated before the course by the Iowa Infant Feeding Attitude Scale (IIFAS). The simple linear regression and multiple linear regression for multivariate analysis was used to determine the influencing factors. Results: A total of 188 fathers were recruited in the study and the mean score of IIFAS was 62.70±6.74. The multiple linear regression model indicated that paternal education, feeding intention and maternal parity were the main influencing factors of paternal breastfeeding attitude. Conclusion: We should pay more attention to promote breastfeeding attitude among fathers with bachelor degree or below, primiparous wife and not positive intention for exclusive breastfeeding, in order to reduce negative impacts, enhance supportive ability and activate the father's role in breastfeeding promotion.

Establishment and evaluation of severe and critical disease training system for oncology nurses in Ningxia

Objective: Establish a severe and critical disease training system for nurses within 5 years of oncology department and evaluate its application effect. Methods: The severe and critical disease cancer training system for oncology nurses was established, and 49 nurses in a tumor hospital in Ningxia were selected and received theoretical training and cultivation of practical ability. The Keshi model was used to evaluate its effect. Results: The overall score of satisfaction for severe and critical disease training was 95.76%, and the satisfaction for trainers, training contents and organizations was 96.14%, 95.56% and 95.41%, respectively. After training, the scores of knowledge and skill increased from (71.88 ±7.05) and (77.24 ±5.41) to (77.24 ±5.40) and (93.88 ±0.75). The total score of nurses' behavior, attitude and post ability (48.92±4.90; 61.37±6.96) was higher than that before training (43.29±6.02; 51.04±9.04), additionally, the overall score of clinical nursing service quality of nurses (95.88±2.52) was higher than that before training (93.92±4.48), the difference was statistically significant (P<0.05). Conclusion: The system of severe and critical disease training system for oncology nurses can enhance the comprehensive ability of nurses, improve the quality of nursing services and ensure the safety of patients.

The influence of positive psychology group counseling on the transition shock of newly graduated nurses

Objective: To explore the influence of positive psychology group counseling on the transition shock of newly graduated nurses. Methods: All the newly graduated nurses in one hospital were included. They were randomly divided into experimental group and control group, which was 34 nurses and 31 nurses respectively. The nurses in the control group participated in the regular business training of the hospital. The nurses in the experimental group received extra five weeks of active psychological group counseling. The newly graduated nurse transition shock scale was applied before and after the intervention. Results: After intervention, the transition shock score of the nurses in the experimental group was lower than that of the control group, and the difference was statistically significant (P<0.001). For the nurses in the experimental group, the total score of transition shock after the intervention was lower than the score before the intervention, and the difference was statistically significant (P<0.001). Conclusion: Positive psychology group counseling can effectively alleviate the impact of newly graduated nurses.

Objective: To investigate the level and influencing factors of pediatric nurses' calling, and analyze its correlation with job satisfaction. Methods: A cross-sectional survey of pediatric nurses from 34 hospitals across China was conducted using the Calling Scale and the Chinese Nurses Job Satisfaction Scale. Results: A total of 547 nurses were enrolled. The mean score of pediatric nurses' calling was 3.27±1.01, which was at a moderate level. Nurses with long working years, without management position, being a senior registered nurse and frequent night shifts were influencing factors associated with lower level of calling. The strong calling group scored higher in overall satisfaction and each subscale than the weak calling group. Conclusion: Hospital managers and educators should increase more calling trainings in the process of career education in order to enhance nurses' awareness of professional sacred and sense of honor, improve satisfaction, and reduce the loss of pediatric nursing staff.

Exploration of practical training base for specialist nurses in medical association based on "ORTCC" Model

Objective: To promote the close cooperation of the specialist medical associations, promote the homogenization of continuous care services in the region, and provide high level and satisfactory health services for the people in the northwest of Sichuan. Methods: Seven specialist nurse training bases were built up. The four teams of doctors, nurses, technicians and information engineers performed the five "1" training modes in the specialized nursing training program before, during and after the training program. Results: The practical ability of the specialist nurses was significantly improved after the training program (P<0.01). Totally 289 specialist practice nurses from 53 hospitals passed the training program. After the training program, 5 county-level hospitals opened specialist nursing clinics. The nurses carried out nursing training more than 100 times, revised more than 60 procedures, and carried out more than 20 new nursing tasks. Conclusion: The establishment of the specialist nurse training bases have provided continuous education program for specialist nurses effectively, which can improve the training system for specialist nurses and the quality of nursing services in northwestern Sichuan. It will promote the close-knit specialist medical association construction.

Objective: To establish an evaluation system of clinical practice competence for Master of Nursing Specialist students based on Mini Clinical Evaluation Exercise. Methods: Based on the original framework of Mini Clinical Evaluation Exercise, the expert consultation questionnaire was initially formed through literature review and experts discussion. Two rounds Delphi experts consultation were conducted on 39 experts. Results: The effective return rates of the 2 rounds questionnaires was 95.24% and 97.50%, respectively, and the experts' authority coefficient was 0.86 and 0.89, respectively. In the second round, the concordance coefficient of experts' opinion on the first and second level item was 0.157 and 0.127, respectively (P<0.001). An evaluation system of clinical practice competence for Master of Nursing Specialist students was established based on Mini Clinical Evaluation Exercise, including 7 first-level items and 55 second-level items. Conclusion: The evaluation system established by this study is scientific and reliable. It could provide a basis for promoting the evaluation and development of clinical practice competence for Master of Nursing Specialist students.

The ophthalmic patient resource center was established to improve the safety management of ophthalmic patients. A nurse-led pre-hospital management team was set up. Nursing assessment, intervention, multi channel health education and evaluation were carried out through the audio-visual information platform. The information registration, telephone follow-up were also carried out with the help of information platform. The pre-hospital management is strengthened. The professional value of nurses is embodied. The patient's medical experience and surgical safety are improved.

Objective: To establish E-coach chronic care model from hospital to home, which integrates the traditional health coaching technology with Internet, and to investigate its feasibility and effects in the management of hypertension. Methods: The E-coach chronic care model was established on the basis of literature review and experts focus group interviews. E-coach chronic care model was performed on outpatients with hypertension, and the blood pressure control rate, medication compliance and participation of offline chronic disease management activities were compared between the E-coach chronic care model group and the routine management group. Results: The blood pressure control rate of patients in the E-coach chronic care model group increased from 27.5% to 61.8% after intervention, which was significantly higher than that of the routine management group (P<0.05). During the intervention period, the participation rate of offline chronic disease management activities in the E-coach chronic care model group was 72.7%, which was higher than that of the routine management group (35.3%), and there was no significant difference in medication compliance between the two groups (P>0.05). Conclusion: The E-coach chronic care model is benefit to improve the control rate of blood pressure and the participation of chronic disease management activities for patients with hypertension and is worthy of clinical application.

Influence of exercise self-efficacy on exercise compliance after discharge of patients receiving hip arthroplasty

Objective: To describe the exercise compliance after discharge and exercise self-efficacy of patients receiving hip arthroplasty and to explore the relationship between them. Methods: The self-designed questionnaire of exercise compliance and the self-efficacy rehabilitation outcome scale were used to investigate a total of 125 patients who were given hip arthroplasty. Results: The average score of exercise compliance after discharge was 58.16±8.34, which was on an intermediate level. The score of exercise self-efficacy had achieved the average level of 62.42±14.92. Multiple linear regression results suggest that exercise self-efficacy is an important influencing factor for exercise compliance after discharge of patients receiving hip arthroplasty. Conclusion: The exercise compliance after discharge of patients receiving hip arthroplasty is low, and the intervention to increase exercise self-efficacy could promote the exercise compliance after discharge.

This paper summarizes the research progress of the current situation and intervention of multi-dimensional caregiver burden for the Primary Family Caregivers (PFCs) of cancer patients. It is suggested that a multi-dimensional social support system should be constructed, including information provision, skills training, humanistic care and financial support at the government level, clinical staff level and family level, in order to reduce the caregiver burden of PFCs of cancer patients, to promote the physical and mental health of PFCs and improve the quality of life of cancer patients and their PFCs.

Objective: To evaluate the effects and feasibility of the application of the Pre-Intra-Hospital Transportation (IHT) Checklist for Critically Ill Patients in Emergency Department (Checklist). Methods: The checklist was developed through evidence summary and Delphi expert argumentation. The incidence of accidents related to IHT and condition change in the two groups were compared and 20 nurses on different levels were selected to record the time spent to fill out the checklist when using the checklist for the first and third month respectively. Results: After applying the checklist, the overall incidence of accidents in the test group was lower than that of the control group (4.0% vs 28.6%, χ2=77.183, P<0.001), and the incidence of accidents relating to instruments, organization coordination, pipelines, liquids and deterioration of condition all decreased as well; the average time for nurses on different levels to fill the checklist in the first month was (2.85±1.05) min, and in the third month was (1.12±0.42) min, showing no statistical difference in the filling time of nurses (P>0.05). Conclusion: The checklist can reduce the incidence of accidents during the IHT of critically ill patients, and the filling time are within the acceptable range, therefore, it can be used by emergency nurses with different working years to carry out pre-IHT check-up of critically ill patients.

Clinical application of best evidence for reasonable physical restraint with ICU adult catheterized patients

Objective: To establish the best evidence of reasonable physical restraint in adults with ICU, and to establish a reasonable physical restraint procedure for adult patients with ICU and evaluate its application. Methods: The systematic search and quality evaluation of the subjects related to the reasonable physical constraints of ICU adult patients were summarized. The best evidences were summarized. Combined with clinical scenarios and professional judgments, 18 evidence-based quality review standards were formulated. The hospital's 1 comprehensive ICU and 5 specialist ICUs were randomly divided into the control group and the implementation group for quality review. Results: There was no significant difference in the rate of restraint and duration of restraint (Days/Thousands of Catheterization) between the two groups (P>0.05) before the edvience applied. After the evidence was applied, the implementation group had lower constraint rate and shorter constraint time (Day/Thousand Tubes Day) than the control group. The difference was statistically significant (P<0.05); the rate of compliance of the analgesic in the implementation group was higher than that before application, the difference was statistically significant (P<0.05). There was no significant difference of the sedation rate and the delirium incidence between the two groups (P>0.05). The degree of anxiety in the implementation group was significantly higher than that in the control group (P<0.05). There was no significant difference in the negative psychological reaction between the family members (P>0.05). The knowledge of the best evidence-related knowledge of doctors and nurses was higher than that before application, and the difference was statistically significant (P<0.05). The implementation of the constrained medical advice the use of the constraint assessment tool, and the signing rate of patient or family consent informed consent form was 100% of the implementation group while the control group still has not issued a doctor's advice, the use of the constraint assessment tool, and the signing of the informed consent form of the patient or family is 100%. There was no significant change in the utilization rate of restraints, the incidence of restrained complications, the incidence of unplanned extubation, and the environmental management of the ICU ward between the two groups. Conclusion: The best evidence for reasonable physical restraint in adult patients with ICU can reduce the restraint rate of adults patients in ICU, shorten the duration of restraint, improve the rate of analgesia, improve the mastery of knowledge of doctors and nurses, and promote the standardization of medical orders and the use of restraint assessment tools to improve the quality of clinical care.

Chronic Heart Failure is the outcome of all kinds of heart diseases. As the incidence of cardiovascular diseases increases, the number of existing and potential patients with Chronic Heart Failure will increase accordingly. Moreover, Chronic Heart Failure is prone to occur recurrently. Transitional care, as an important means to control the recurrence and deterioration of the disease, is of great significance to the rehabilitation of patients. This article reviewed the patterns of transitional care for patients with Chronic Heart Failure, in order to provide basis for future research.

Objective: To compare the differences of the utilization in perinatal care services among women with different census register types in Beijing. Methods: The data was collected from the records of the perinatal care system in a community health service center in Beijing. The chi-square test was used to explore the differences of the utilization in perinatal health care services among different household registered women. Results: There were significant differences in the utilization in nine aspects among different household registration women, including the start time of taking folic acid, the gestational week of the first prenatal examination and so on (P<0.05). Those the pregnant and husband were both non-residents used the perinatal care services worse, and those the pregnants were non-residents but the husbands were residents were with the best utilization of postnatal care services (P<0.05). Conclusions: Those the pregnant and husband were both non-residents used the perinatal care services worst, and they should be paid special attentionin the future.

Meta-analysis of the feasibility of elective hepatectomy without indwelling nasogastric tube or early removal of nasogastric tube

Objective: To investigate the effect of no nasogastric intubation during operation or early removal of nasogastric intubation after operation for patients with elective hepatectomy. Methods: The database of the PubMed, Web of Science, Cochrane Library, CNKI, Wanfang Data and CBM database were screened by two researchers. The Meta-analysis was performed using RevMan5.3 software. Results: A total of 11 articles were included in the study. Totally 826 patients with no nasogastric tube during operation or removal of nasogastric tube within 24 hours after operation were included in the observation group, while 842 with nasogastric tube removal after anal exhaustion were included in the control group. The incidence of postoperative throat pain and pulmonary complications in the observation group were significantly lower than the control group respectively (OR=0.16, 95% CI=0.10-0.23, P<0.05; OR=0.57, 95% CI=0.40-0.81, P<0.05 ). The incidences of postoperative abdominal distention (OR=0.90, 95% CI=0.65-1.23, P>0.05), nausea and vomiting (OR=0.72, 95% CI=0.40-1.30, P>0.05) and other surgical complications (OR=0.76, 95% CI=0.58-1.00, P=0.05) in the observation group were less than the control group, but the differences between the two groups were not statistically different. There were no significant differences between the two groups in postoperative anal exhaust time, feeding time and postoperative hospitalization time. Conclusion: For patients with elective hepatectomy, it seems feasible to have no nasogastric tube during operation or early removal of the nasogastric tube after operation.

The important goal of international critical care medicine is to promote Family-Centered Care (FCC) in Intensive Care Units (ICUs), increase the participation of patients' families, and ultimately improve patient safety. The Society of Critical Care Medicine (SCCM) has recruited experts to develop evidence-based guidelines on clinical issues in its implementation. This paper interpretes the guidelines for FCC to help health care professionals to understand the implementation method of FCC from five aspects: family presence in the ICU, family support, communication with family members, use of specific consultations and ICU team members, operational and environmental issues. It is expected to provide evidence-based strategies to standardize and solve specific problems in FCC practice, optimize the support for patients' family, and improve the patient safety and the quality of care.

Objective: To analyze the current status of the curriculum design for doctoral education in nursing in foreign countries and to provide reference for optimizing the curriculum design for nursing doctoral education in China. Methods: By means of the network and field survey, the curricular information of eight nursing schools in Northern America were collected and analyzed. Results: The curriculum design for nursing doctoral education highlighted nursing philosophy and theory, and advanced research methods. The courses mainly include five major categories, i.e. professional basic ability, scientific research, research methods, interdisciplinary frontier and academic writing. Various methods of teaching and evaluation were used flexibly and much importance was attached to the cultivation of scientific research ability, multidisciplinary cooperation ability and innovation ability training. Conclusion: China's nursing doctoral education can learn from foreign curriculum setting experience, update the curriculum design concept, refine the cultivation ability requirements, define the core courses of nursing, optimize teaching and evaluation methods, and deepen the research fields to further improve the quality of nursing doctoral students.
