The exploration and practice of carrying out the hospice co-care model in cancer specialist hospitals

Abstract: In the exploration of the hospice care suitable for cancer specialist hospitals, our hospital has formed a hospice co-care model, that is, the hospice co-care team and the original treatment team jointly provide services and build flexible hospice wards and the medical conjoined unit mode. Collaborate medical units share palliative care bed and co-care mode. The content of this mode includes multidisciplinary team symptom management, comfort care, spiritual care, grief counseling, psychological and social support, etc. Through the implementation of the hospice co-care model, we provide multiple medical decisions for the end-stage cancer patients and their family members, and take care of their feelings throughout the course of the disease. It has accumulated a lot for the practice of hospice care and explored a hospice co-care model suitable for the national condition.
