Abstract: This paper introduced the definition and concept of case management, presented the implementation and application of models in Cardiovascular Disease case management around the world. Moreover, under influence of the Chinese new medical reform, this paper discussed the key points that were essential to the implementation process of the case management models in the field of Cardiovascular Disease, including dissemination of the concept in case management, preparation and organization of case managers, that would provide evidence and reference for the establishment of new nursing models in Cardiovascular Disease management in China.
