基于倾向性评分匹配法评价岗位设置对专科护士实践水平的影响

Evaluation of the influence of full-time posts setting on the practice performance of specialist nurses based on Propensity Score Matching

摘要: 目的:调查上海市专科护士专职岗位设置情况,并分析岗位设置对专科护士实践水平的影响。方法:调查上海市16家医院的612名专科护士的岗位情况及其实践水平,采用倾向性评分匹配法控制混杂因素,比较岗位设置对专科护士实践水平的影响。结果:上海市有专职岗位的专科护士的比例为9.5%,有专职岗位的专科护士在三级综合性医院、内外科和监护室工作的比例较高。有专职岗位的专科护士的年龄、工作年限均高于兼职的专科护士,主管护师及以上的人数比例多于兼职的专科护士(P<0.05)。经倾向性评分匹配法均衡两者的基线资料后,发现有专职岗位的专科护士在临床实践和协调2个维度的实践水平与兼职的专科护士的差异无统计学意义(P>0.05),在系统优化及教育、科研和专业引领3个维度的实践水平均高于兼职岗位的专科护士,差异有统计学意义(P<0.05)。结论:上海市专科护士有专职岗位的比例较小,主要分布在三级综合性医院,其中以内外科及监护室为主;专职岗位倾向于工作年限长、职称高的专科护士。同时,设置专职岗位也有利于专科护士提升实践水平,更好地发挥作用。

Abstract: Objective: To explore the influence of full-time posts setting on the practice performance of specialist nurses. Methods: A total of 612 specialist nurses from 16 hospitals in Shanghai were included in this survey. The Propensity Score Matching (PSM) was applied to control the confounding factors. The influence of different posts setting on the practice performance of specialist nurses was explored. Results: The proportion of specialist nurses with full-time posts was 9.5%. The specialist nurses were distributed mainly in tertiary hospitals, general hospitals, internal medicine, surgery and intensive care units. The specialist nurses with full-time posts were significantly older and had longer working years and higher proportion of nurse-in-charge titles than those with part-time posts (P<0.05). There was no significant difference in the practice performance of clinical practice and coordination ability between the specialist nurses with full-time posts and those with part-time posts (P>0.05). The practice performance of the specialist nurses with full-time posts were significantly higher in systems optimization, education, research and leadership than that of the specialist nurses with part-time posts (P<0.05). Conclusion: The proportion of specialist nurses with full-time posts in Shanghai is relatively small. They are mainly distributed in tertiary general hospitals, and work in the departments of internal medicine, surgery and intensive care units. The specialist nurses with more work experiences and higher professional titles are easier to get full-time posts than the others. The establishment of full-time posts can help specialist nurses to improve their practice performance in nursing.
