62所医院的医院聘用医疗护理员人力配置及管理现况调查

Current status of human resource allocation and management of medical nursing assistants employed by hospital in 62 hospitals

摘要: 目的:调查医院聘用的医疗护理员的人力配置及管理现况,为医疗护理员的使用及管理提供决策参考。方法:于2023年7月—10月,采用便利抽样法在我国东、中、西、东北地区共选取62所医院,使用自行编制的医院聘用医疗护理员人力配置与管理现况调查问卷进行调查。结果:62所医院的医院聘用医疗护理员人数与实际开放床位数之比的中位数为0.036,医院聘用的医疗护理员人数与护士人数之比的中位数为0.055,不同等级医院聘用医疗护理员配置比例差异无统计学意义(P>0.05);27所(43.5%)医院有全院医疗护理员人数配置标准,但各医院标准不同;54所(87.1%)医院通过与第三方公司签订合同以劳务派遣的方式聘用医疗护理员;42所医院提供了聘用护理员的具体成本,中位数为每人3?870.0元/月。各医院聘用的医疗护理员的工作内容主要以清洁照护(80.6%)和生活照护(79.0%)为主;45所(72.6%)医院采取“一对多”照护模式,按5天/周、8?h/天给医疗护理员排班的医院较多,但也有一部分医院的医疗护理员排班时间延长。62所医院均对聘用的医疗护理员进行培训;33所(53.2%)医院要求上岗前必须取得护理员资格证书。62所医院均由医院主导医疗护理员工作质量管理,25所(40.3%)医院由医院护理部制定医疗护理员工作质量评价标准。结论:医院成为医疗护理员工作质量管理的主体,医疗护理员在各医院的工作内容基本一致、工作模式多样,需要逐步探索医院聘用及配置医疗护理员的标准。

Abstract: Objective: To investigate the current status of human resource allocation and management of medical nursing assistants employed by hospital in China, providing a reference of decision-making to promote their management in clinical practice. Methods: A cross-sectional survey was conducted from July to October 2023. Sixty-two hospitals located in the east, middle, west, and northeast regions of China were selected through convenience sampling. A self-designed questionnaire was used to collect data on the allocation and management of medical nursing assistants employed by hospital. Results: The median ratio of the number of medical nursing assistants employed by hospital to hospital beds among 62 hospitals was 0.036, and the median ratio of medical nursing assistants employed by hospital to nurses was 0.055, with no statistically significant difference across hospitals of different grades (P>0.05). Allocation standards for medical nursing assistants were reported in 27 hospitals (43.5%), although these standards varied across hospitals. Most hospitals (87.1%) employed medical nursing assistants through labor dispatch companies, and among the 42 hospitals that provided cost information, the median costs was 3870.0 CNY per person per month. The main responsibility of medical nursing assistants employed by hospital included cleaning care (80.6%), and living care (79.0%). A "one-to-many" working mode was adopted in 72.6% of hospitals, with most medical nursing assistants working 5 days per week and 8 hours per day, although longer working hours were reported in some cases. All hospitals provided training for medical nursing assistants employed by hospital, and 53.2% required certification before starting to work. Work quality supervision was implemented in all hospitals, and 40.3% of hospitals had established quality evaluation criteria by nursing department. Conclusion: Hospitals have become the main body of quality management for medical nursing assistants. The work content of medical nursing assistants in various hospitals is basically consistent, and their work modes are diverse. It is necessary to gradually explore scientific and reasonable allocation standards of medical nursing assistants.
