慢性病患儿接受侵入性操作身心感受及需求的质性研究

Qualitative study of children with chronic diseases undergoing invasive operation for physical and mental experience and needs

摘要: 目的:了解慢性病患儿接受侵入性操作的身心感受及需求。方法:以质性研究中的现象学研究方法为指导,采用目的抽样法选取16例曾接受过侵入性操作的慢性病患儿为研究对象,对其进行半结构化访谈。结果:患儿接受侵入性操作中存在身体不适和复杂的心理反应,身体不适包括疼痛与体位限制的不适,复杂心理反应包括紧张与恐惧、茫然与无助、悲伤与压抑、信任与焦虑。操作前患儿存在信息和同伴支持的需求;操作中患儿存在环境和舒适、尊重、沟通及转移注意力的需求;操作后患儿存在信息及爱与归属的需求。结论:未来应以患儿自身感受及需求为重要依据,积极探索适合患儿的个性化、多样化干预措施,以减少侵入性操作为患儿所带来的负性体验,提高操作依从性,进一步提高护理质量。

Abstract: Objective: To explore the physical and mental experience and needs of children with chronic diseases undergoing invasive operation. Methods: Under the guidance of phenomenological methodology, 16 children with chronic diseases who had undergone invasive operation were selected for the purpose of semi-structured interviews. Results: During the invasive procedures, the children had physical discomfort and complex psychological reactions, including pain and position limitation, and complex psychological reactions including tension and fear, loss and helplessness, sadness and depression, trust and anxiety. Children need information and peer support before invasive procedures. During operation, children had the need of comfort environment, respect, communication and distraction. After the operation, the children had the need of information, love and belonging. Conclusion: Nursing interventions should be based on the child's own feelings and needs. Nurses should pay attention to children's own characteristics and preferences, and actively explore personalized nursing care in order to improve the children's experience.
