手臂输液港技术在770例乳腺癌患者中应用的前瞻性研究

Prospective study of arm port in 770 patients with breast cancer

摘要: 目的:观察手臂输液港技术在乳腺癌患者中应用的相关并发症。方法:将2016年5月至2019年9月收治于我病区的770例置入手臂输液港的女性乳腺癌化疗患者作为研究对象,随访并记录与手臂输液港相关的并发症,分析并发症的发生情况。结果:770例置入手臂输液港的患者出现相关并发症为34例,并发症发生率为0.218例/千携港日。其中静脉导管相关性感染和静脉血栓形成为主要并发症,分别是13例和8例,其他并发症为心律失常、导管移位、堵管、持续疼痛。不同体质量指数(Body Mass Index,BMI)以及不同置入时间的患者静脉导管相关性感染和静脉血栓的发生率有统计学差异(P<0.05),不同TNM分期的患者静脉导管相关性感染的发生率有统计学差异(P<0.05)。结论:手臂输液港技术是安全可行的,其并发症发生率较低。对于乳腺癌患者,手臂输液港是较好的化疗静脉通道工具。

Abstract: Objective: This prospective study investigated device-related complications of arm port implanted in the upper arm of the patients with breast cancer. Methods: Between May 2016 and September 2019, 770 women with breast cancer who received chemotherapy and implanted a arm port in the upper arm were included. Device-related complications were collected and influencing factors were analyzed for major complications. Results: The incidence of complications in 770 patients with arm port was 34, and the rate of complications was 0.218‰ catheter-days. Among them, venous catheter-related infection and catheter-related thrombosis were the most common complications, occurring in 13 and 8, respectively. Other complications were arrhythmia, catheter shift, tube blockage, and persistent pain. Patients with higher Body Mass Index(BMI) and longer implantation times had significantly higher rate of infection and venous thrombosis (P<0.05). Patient's TNM staging affected infection (P<0.05). Conclusion: The technique of arm port is safe and feasible, with a low rate of complications. For breast cancer patients, arm port is a better tool for intravenous chemotherapy.
