肝移植病房多重耐药菌感染防控的持续质量改进效果观察

Effects of continuous quality improvement on prevention and control of Multi-drug Resistant Organisms infection in the liver transplantation ward

摘要: 目的:研究多学科协作联合危急值管理模式用于肝移植病房多重耐药菌(MDROs)感染防控的效果。方法:以2017年7月至2020年7月某医院肝移植病房患者为研究对象,自2018年7月开始将多学科协作(MDT)联合危急值管理模式用于肝移植病房MDROs感染的防控,并从2019年7月起进行持续质量改进,比较实施前、实施后、持续质量改进后肝移植病房住院患者MDROs感染率、隔离措施落实率和漏报率。结果:实施前、实施后和持续质量改进后肝移植病房患者MDROs感染率分别为2.88%、1.84%、1.39%,呈下降趋势(P<0.05);肝移植病房MDROs接触隔离措施整体落实率呈上升趋势(P<0.05),其中手卫生依从性、手卫生正确率、穿隔离衣依从性、外出检查告知依从性等明显上升(P<0.001);实施前、实施后和持续质量改进后MDROs感染漏报率分别为28.57%、16.00%、4.76%,呈下降趋势(P<0.05)。结论:建立MDT感染管理小组,纳入危急值管理模式,加强院科两级督查,可明显提高肝移植病房MDROs接触隔离措施落实率、降低肝移植病房MDROs的感染率和漏报率。

Abstract: Objective: To explore the effects of multidisciplinary collaboration combined with the critical value management mode on the prevention and control of Multi-Drug Resistant Organisms (MDROs) infection in the liver transplantation ward. Methods: Patients in the liver transplantation ward of a hospital from July 2017 to July 2020 were selected as the research subjects. MDT combined with the critical value management mode was applied to the prevention and control of MDROs infection from July 2018, and continuous quality improvement was carried out one year later. Indicators including the infection rate of MDROs, the implementation rate of isolation measures and the omission rate of report of inpatients were compared before and after the implementation, and after the application of continuous quality improvement. Results: The infection rate of MDROs before the implementation, after the implementation and after continuous quality improvement was 2.88%, 1.84% and 1.39%, respectively, showing a decreasing trend with statistically significant differences (P<0.05). The overall implementation rate of MDROs prevention and control measures increased (P<0.001), and indicators like the compliance and accuracy rate of hand hygiene, the compliance of wearing isolation clothes, the compliance of reporting outside examination information and so on were significantly improved, with statistical significance (P<0.001). The omission rate of MDROs infection was 28.57%, 16.00% and 4.76% before the implementation, after the implementation and after continuous quality improvement, respectively, with statistically significant differences (P<0.05). Conclusion: The establishment of a multidisciplinary infection management team, the inclusion of the critical value management mode, and the strengthening of the two-level supervision between hospital and department can significantly improve the implementation rate of multi-drug resistant bacteria isolation measures in the liver transplantation ward to effectively reduce the infection rate and the omission rate of MDROs cases.
