肺癌首次化疗患者癌因性疲乏及其影响因素的纵向研究

摘要: 目的:调查肺癌化疗患者在不同治疗阶段癌因性疲乏的发生状况,并分析发生癌因性疲乏的影响因素。方法:采用一般资料调查表、癌症疲乏量表调查2020年12月至2021年12月入住上海某医院的249名患者首次化疗前、第1~6次化疗后1周7个时间点癌因性疲乏状况,采用Logistic回归分析癌因性疲乏的影响因素。结果:肺癌首次化疗患者癌因性疲乏发生率为44.9%~50.4%。疲乏程度以轻度为主,随着化疗进行疲乏程度呈现上升趋势,第4次化疗后疲乏程度最严重。职业、居住方式、家庭人均月收入、BMI、慢性病(脑卒中、高血压)、咳嗽情况、呕吐情况、红细胞计数在不同化疗阶段为肺癌化疗患者癌因性疲乏的影响因素(P<0.05)。结论:肺癌化疗患者不同治疗阶段癌因性疲乏的影响因素不同,应在不同治疗时期从生理、心理、社会多维度给予患者不同关注与干预,逐步促进患者康复。

Abstract: Objective: To investigate the incidence of Cancer-Induced Fatigue in lung cancer patients with chemotherapy at different stages of treatment, and to analyze the factors related to the occurrence of Cancer-Induced Fatigue. Methods: The general data questionnaire and the cancer fatigue scale were used to evaluate the cancer fatigue status of 249 patients in a hospital in Shanghai from December 2020 to December 2021 before the first chemotherapy and the first to six chemotherapy at seven-time points, and the influencing factors of cancer fatigue were analyzed by multivariate logistic regression. Results: The incidence of Cancer-Induced Fatigue in lung cancer patients undergoing chemotherapy was 44.9% to 50.4% for the first time. The degree of fatigue was mainly mild, and the degree of fatigue showed an upward trend with chemotherapy. The degree of fatigue was the most severe after the fourth chemotherapy. Occupation, mode of residence, household per capital monthly income, BMI index, chronic diseases (stroke, hypertension), cough, vomiting, and red blood cell count were factors influencing cancer fatigue in lung cancer patients at different stages of chemotherapy (P<0.05). Conclusion: The influencing factors of Cancer-Induced Fatigue in different treatment periods of lung cancer chemotherapy patients are different, and clinically patients should be given different attention and interventions from physiological, psychological and social dimensions at different treatment periods, and gradually promote patient recovery.
