肿瘤患者PICC导管相关血流感染风险预测模型的构建

摘要: 目的:分析肿瘤患者PICC留置期间发生导管相关血流感染(CRBSI)的危险因素并建立预测模型。方法:回顾性分析2017年1月至2019年12月在上海市某三级甲等医院血管通路护理门诊进行PICC置管、维护并拔管的11?901例肿瘤患者资料,用Logistic回归分析发生CRBSI的危险因素,基于危险因素建立列线图预测模型并进行内部验证。结果:PICC留置期间CRBSI的发生率为0.39%。回归分析显示皮炎、导管相关性血栓、局部感染、渗液为CRBSI的独立危险因素。训练集列线图预测模型预测曲线趋势接近标准曲线,C-index为0.825(95%CI:0.78~0.91),验证集ROC曲线下面积为0.950(95%CI:0.917~0.996),验证集预测曲线趋势接近标准曲线。结论:肿瘤患者PICC留置期间的皮炎、导管相关性血栓、局部感染、渗液是CRBSI的独立危险因素,本研究建立的列线图模型具有良好的预测能力和区分度,有利于筛查高风险患者。

Abstract: Objective: To analyze the risk factors of Catheter-Related Bloodstream Infection (CRBSI) in cancer patients during PICC retention, and to establish a prediction model. Methods: Totally 11901 cancer patients who underwent PICC insertion, maintenance and removal in the vascular access nursing clinic of a tertiary grade A hospital in Shanghai from January 2017 to December 2019 were retrospectively analyzed. Logistic regression were used to analyze the independent risk factors of CRBSI. Based on the risk factors, a nomogram prediction model was established, and bootstrap method was used for internal validation. Results: The incidence of CRBSI during PICC retention was 0.39%. Multivariate analysis results showed that dermatitis, catheter-related thrombosis, local infection and exudation were independent risk factors for CRBSI. The trend of the prediction curve was close to the standard curve, and the consistency index was 0.825 (95%CI: 0.78-0.91). The ROC curve had an area under the curve of 0.950 (95%CI: 0.917-0.996), and the trend of the prediction curve of the validation group is close to the standard curve. Conclusion: Dermatitis, catheter-related thrombosis, local infection and exudation during PICC retention were independent risk factors for CRBSI. The nomogram model established in this study has good predictive ability and differentiation, which is conducive to clinical screening of high-risk patients.
