多学科协作居家营养管理策略在维持性血液透析患者中的应用

Application of multidisciplinary team based-home nutrition management in patient with maintenance hemodialysis

摘要: 目的:分析多学科协作居家营养管理策略在维持性血液透析患者中的应用效果。方法:采用便利抽样法,选取2019年12月至2020年4月在杭州市某医院肾内科接受血液透析治疗的患者67例,随机分为实验组(33例)和对照组(34例)。对照组患者采用常规饮食管理,实验组在对照组的基础上采用多学科协作居家营养管理策略,比较两组患者干预前后的营养指标。结果:干预后,实验组患者的血红蛋白、白蛋白水平和营养良好率高于对照组(P<0.05),重度营养不良率、血清磷和免疫反应性甲状旁腺激素浓度低于对照组(P<0.05),体质量改变、胃肠道症状、饮食改变、肌肉消耗评分和改良主观全面营养评估总分均低于对照组(P<0.05),营养不良炎症评分低于对照组(P<0.05)。结论:多学科协作居家营养管理策略对于改善维持性血液透析患者的营养状况、钙磷代谢和微炎症状态,降低营养不良的发生率有显著作用,值得临床推广应用。

Abstract: Objective: To explore the efficacy of multidisciplinary home nutrition management for maintenance hemodialysis patient. Methods: A total of 67 patients who received hemodialysis in the Department of Nephrology from December 2019 to April 2020 in a hospital in Hangzhou were enrolled and randomly divided into intervention group (33 cases) and control group (34 cases). Those in the control group were with regular diet, while those in the experimental group were with home nutrition management based on multidisciplinary cooperation besides regular diet. The nutritional indicators were compared between the two groups before and after intervention. Results: After the intervention, the levels of hemoglobin and albumin in the experimental group were significantly higher than those in the control group (P<0.05), the rate of good nutrition in the experimental group was significantly higher and the rate of severe malnutrition was significantly lower than that in the control group (P<0.05). The levels of serum phosphorus and iPTH, weight change, the changes of gastrointestinal symptoms, dietary changes, muscle consumption score and total score, and the score of malnutrition and inflammation in the experimental group were significantly lower than those in the control group (P<0.05). Conclusion: Home nutrition management based on multidisciplinary cooperation has a significant effect on improving the nutritional status, calcium and phosphorus metabolism and microinflammation status of maintenance hemodialysis patient, and reducing the incidence of malnutrition.
