成人动脉血气分析临床操作实践现况调查

Investigation of clinical practice of arterial blood gas analysis in adults

摘要: 目的:调查2017年发布的《动脉血气分析临床操作实践标准》临床实践现状,为进一步规范成人动脉血气分析临床操作实践提供借鉴。方法:采用便利抽样法,于2022年4月12日至5月31日,采用自行设计的调查问卷对全国134家医疗机构内的医护人员进行成人动脉血气分析操作实践现况调查。结果:共回收有效问卷500份,28.2%的受访者首选2%葡萄糖酸氯己定乙醇溶液作为皮肤消毒剂;采血时90.4%首选部位为桡动脉,进针角度略有不同;仅15.0%的导管采血受访者会移除导管死腔体积3倍的混合血液;73.8%的受访者在采血后正确进行了标本混匀操作;76.2%的受访者能在30分钟内完成上机检测;有8.6%的受访者因动脉采血操作而发生针刺伤。结论:目前医护人员动脉血气分析皮肤消毒方法、采血穿刺部位、进针角度存在差异,动脉血气标本收集、运输及保存环节质量控制尚须优化,动脉血气分析相关培训和考核不足,职业防护意识较薄弱,可通过加强成人动脉血气分析操作知识和技能培训、采集过程的质量控制等,提高动脉采血的成功率,保证检测结果的准确性。

Abstract: Objective: To investigate the clinical practice status of the Clinical Practice Standards for Arterial Blood Gas Analysis published in 2017, and to provide reference for further standardizing clinical practice for arterial blood gas analysis in adults. Methods: From April 12 to May 31, 2022, a self-designed questionnaire was used to investigate the practice status of adult arterial blood gas analysis among medical staff from 134 medical institutions in China by convenience sampling. Results: A total of 500 valid questionnaires were collected, and 28.2% respondents preferred 2% chlorhexidine gluconate ethanol solution as skin disinfectant. In clinical arterial blood gas sampling, 90.4% of the preferred site were radial artery, and the needle entry angles were slightly different. Only 15.0% removed three times the dead space volume of mixture blood; 73.8% correctly mixed the specimens after blood collection; 76.2% could complete the test within 30 minutes. There were 8.6% of the respondents who suffered needle injuries due to arterial blood collection operation. Conclusion: There are differences in the disinfection methods of arterial blood gas analysis, puncture sites for blood collection, and needle entry angles among medical staff. The quality control of arterial blood gas sample collection, transportation, and preservation needs to be optimized. The training and examination related to arterial blood gas analysis are insufficient, and the awareness of occupational protection is weak. The success rate of arterial blood collection can be improved by strengthening the training of adult arterial blood gas analysis operation knowledge and skills, and the quality control of the collection process, to ensure the accuracy of test results.
