奥塔戈运动对膝关节置换术后患者膝关节康复影响的Meta分析

Effects of Otago Exercise Program on the knee rehabilitation of patients after knee arthroplasty: a Meta-analysis

摘要: 目的:评价奥塔戈运动对膝关节置换术后患者膝关节康复情况的影响。方法:计算机检索CNKI、WanFang Data、VIP、FMRS、PubMed、Web of Science和Cochrane Library数据库,检索采用奥塔戈运动对膝关节置换术后患者进行康复锻炼的随机对照试验,检索时限从建库至2021年4月30日。由2名研究者独立完成文献筛选、资料提取和纳入文献的方法学质量评价,采用RevMan?5.3软件进行Meta分析。结果:共纳入6项随机对照试验。Meta分析结果显示,采用奥塔戈运动干预后,膝关节置换术后患者膝关节屈曲度[SMD=10.71(7.92,13.50),P<0.05]、纽约特种外科医院膝关节评分[SMD=7.66(5.55,9.77),P<0.05]、跌倒效能量表评分[SMD=3.87(2.02,5.71),P<0.05]均高于常规康复锻炼,但Berg平衡量表评分[SMD=1.96(-1.74,5.67),P>0.05]差异无统计学意义。结论:奥塔戈运动可促进膝关节置换术后患者膝关节功能恢复,减少跌倒的发生,且未见不良事件报道。但受纳入文献数量和质量的限制,还需要更多高质量研究进一步验证。

Abstract: Objective: To evaluate the effect of Otago Exercise Program on the knee rehabilitation of patients after knee arthroplasty. Methods: CNKI, WanFang Data, VIP, FMRS, PubMed, Web of Science and Cochrane Library databases were searched, retrieved randomized controlled trials of rehabilitation exercise after knee arthroplasty using Otago Exercise Program. Retrieval time limit was from the establishment of the database to April 30th 2021. Two researchers independently completed the literature screening, data extraction and methodological quality evaluation of the included literature. RevMan 5.3 software was used for Meta-analysis. Results: A total of 6 randomized controlled trials were included. The Meta-analysis results showed that the knee flexion after Otago Exercise Program [SMD=10.71(7.92, 13.50), P<0.05], hospital for special surgery knee score [SMD=7.66 (5.55, 9.77), P<0.05], and Modified Falls Eglcacy Scale score [SMD=3.87 (2.02, 5.71), P<0.05] were higher than conventional rehabilitation exercises, but the difference of Berg Balance Scale score [SMD=1.96(-1.74, 5.67), P>0.05] was not statistically significant. Conclusion: Otago Exercise Program can promote the recovery of knee joint function in patients after knee arthroplasty and reduce the occurrence of falls. There were no reports of adverse events. However, the included studies were limited, need to be further verified by more high-quality studies.
