间歇性气压治疗对乳腺癌相关淋巴水肿干预效果的Meta分析

Effect of Intermittent Pneumatic Compression therapy on Breast Cancer-Related Lymphedema: a systematic review

摘要: 目的:评价间歇性气压治疗在预防和治疗乳腺癌相关淋巴水肿方面的效果。方法:检索从建库至2021年6月Cochrane Library、Medline(Ovid)、Embase(Ovid)、Web of Science、EBSCO、中国生物医学文献数据库、中国知网、万方和维普数据库中关于间歇性气压治疗防治乳腺癌相关淋巴水肿的随机对照试验。由2名研究者对文献进行严格质量评价及资料提取,并对符合标准的研究进行Meta分析。结果:共纳入13个随机对照试验。Meta分析结果显示,间歇性气压治疗对预防乳腺癌相关淋巴水肿发生有显著作用,但干预组和对照组患者患肢水肿程度和关节活动度的改善情况差异无统计学意义。结论:间歇性气压治疗能有效预防乳腺癌相关淋巴水肿的发生,但在治疗淋巴水肿,改善患肢活动功能和患者主观感受方面尚无明显优势。

Abstract: Objective: To evaluate the effectiveness of Intermittent Pneumatic Compression therapy in the prevention and treatment of Breast Cancer-Related Lymphedema. Methods: We searched the Cochrane Library, Medline (Ovid), Embase (Ovid), Web of Science, CBM, CNKI, WanFang Data, and VIP databases for randomized controlled trials on Intermittent Pneumatic Compression therapy for the prevention and treatment of Breast Cancer-Related Lymphedema from the time of database construction to June 2021. Two researchers conducted quality evaluation and data extraction. Meta-analysis was performed on studies that met the criteria. Results: A total of 13 randomized controlled trials were included. Meta-analysis results showed that Intermittent Pneumatic Compression therapy had a significant effect on preventing Breast Cancer-Related Lymphedema, but there was no significant difference in the degree of lymphedema and improvement in joint mobility of the affected limbs between the intervention and control groups. Conclusion: Intermittent Pneumatic Compression therapy can effectively prevent the occurrence of Breast Cancer-Related Lymphedema, but it has no obvious advantageous effect in treating lymphedema, improving the mobility of affected limbs and patients' subjective feelings.
