恶性肿瘤伤口患者症状生活困扰现状及其影响因素分析

Symptoms intrusiveness situation and the influencing factors in patients with Malignant Fungating Wounds

摘要: 目的:调查恶性肿瘤伤口患者症状生活困扰水平并分析其影响因素。方法:采用便利抽样法,于2019年1月至2021年5月在南宁市某三级甲等医院选取132名恶性肿瘤伤口患者作为研究对象,使用一般资料调查表和症状生活困扰评估量表等工具进行调查,评估恶性肿瘤伤口患者症状生活困扰水平并运用多元线性回归分析其影响因素。结果:恶性肿瘤伤口患者症状生活困扰总分为(55.42±14.21)分,其中功能状态维度为(19.50±4.73)分、社会关系维度为(17.50±6.12)分、心理健康维度为(18.42±4.55)分。多元线性回归结果显示,性别、伤口渗液量、伤口气味、伤口瘙痒、换药频率、心理痛苦是恶性肿瘤伤口患者症状生活困扰的影响因素(P<0.05),可解释总变异的79.7%。结论:恶性肿瘤伤口患者症状生活困扰水平较高,应关注患者伤口或肿瘤相关症状,帮助患者做好症状管理,引导患者积极应对现存症状和疾病,予以心理支持与疏导,提升患者生活质量。

Abstract: Objective: To investigate the level of symptom intrusiveness in patients with Malignant Fungating Wounds and analyze the influencing factors. Methods: From January 2019 to May 2021, 132 patients with Malignant Fungating Wounds were selected as the research subjects by convenience sampling method in a tertiary grade A hospital in Nanning. The general information questionnaire and the Symptom Intrusiveness Rating Scale and other tools were used to evaluate the level of symptom intrusiveness of patients with Malignant Fungating Wounds. Using multiple linear regression to analyze the influencing factors. Results: The total score of symptom intrusiveness in patients with Malignant Fungating Wounds was (55.42±14.21). The scores of the dimensions were functional status (19.50±4.73), social relationship (17.50±6.12), mental health (18.42±4.55). Multiple linear regression results showed that gender, wound exudation amount, wound odor, wound itching, dressing change frequency and psychological distress were the influencing factors of symptom intrusiveness of patients with Malignant Fungating Wounds (P<0.05), accounted for 79.7% of the total variation. Conclusion: Patients with Malignant Fungating Wounds have higher symptom intrusiveness level. Nurses should pay attention to the patient's wound or tumor-related symptoms, to help patients manage their symptoms, guide patients to actively deal with existing symptoms and illnesses, give psychological support and counseling, improve patients' quality of life.
