低年资护士护理专业韧性发展模型:一项扎根理论研究

Professional resilience development model in junior nurses: a grounded theory study

摘要: 目的:研究低年资护士护理专业韧性的建构路径及其影响要素,为开展干预研究和实践指导提供理论依据。方法:对17名低年资护士进行半结构式访谈,采用Charmaz建构主义扎根理论的范式收集和分析资料。结果:建构了低年资护士护理专业韧性发展模型——松树模型(Pine Tree Model,PTM),阐释了护士专业韧性的发展实质是其专业成长的过程,共有4个可以相互转化的发展路径:高水平韧性-事业型、中高水平韧性-职业型、中低水平韧性-工作型和低水平韧性-离职型,以及影响护理专业韧性的5个主题和16个副主题因素。结论:护理专业韧性的发展是一个分层级、动态变化的过程,受到个体特质、专业特质、人际关系、机构因素以及社会因素的影响。本研究为护理专业韧性测评工具的发展提供了概念框架,同时也为医院提升低年资护士专业韧性水平和专业坚守度提供了理论依据。

Abstract: Objective: To explore the constructive process and influencing factors of nursing professional resilience development in junior nurses. Methods: Semi-structured interviews were conducted among 17 junior nurses, and Charmaz's constructing grounded theory paradigm was used to guide the data collection and data analysis. Results: The Pine Tree Model (PTM), a nursing professional resilience development model in junior nurses, was constructed. This model presented that the essence of the nursing professional resilience development was the nursing profession growing process with four developmental paths: high level resilience-profession path, medium-high level resilience-career path, medium-low level resilience-job path and low level resilience-resignation path. Moreover, five themes and 16 sub-themes affecting nursing professional resilience were identified. Conclusion: The development of nursing professional resilience is a multilevel and dynamic process, which is influenced by individual attributes, professional attributes, interpersonal relationships, institutional level factors and social level factors. This study provides a conceptual framework for the development of nursing professional resilience measurement, and also provides a theoretical preference to improve the professional resilience level and retention rate among junior nurses.
