中青年冠心病患者职业紧张对冠状动脉病变严重程度的影响

The association between job stress and the severity of coronary artery disease in young and middle-aged patients with Coronary Heart Disease

摘要: 目的:分析中青年冠心病患者职业紧张对冠状动脉病变严重程度的影响。方法:采用方便抽样选取2020年1月—12月于北京某三级医院行冠状动脉造影术的419例中青年患者,根据造影结果是否诊断为冠心病将其分为冠心病组和对照组。采用Gensini评分评价冠脉病变严重程度,将冠心病组分为轻度病变组、中度病变组和重度病变组。采用一般资料调查表、付出-回报失衡问卷对患者进行调查。采用Spearman相关、等级Logistic回归分析探讨职业紧张对冠脉病变严重程度的影响。结果:不同冠状动脉病变组间外在付出、付出-获得不平衡指数(ERI)和内在投入差异均具有统计学意义(P<0.05)。外在付出、ERI值和内在投入与Gensini积分均呈正相关(P<0.05)。外在付出(OR=1.215;95%CI:1.115,1.324)、ERI值(OR=9.300;95%CI:2.716,31.849)和内在投入(OR=1.244;95%CI:1.137,1.361)是冠脉病变严重程度的独立危险因素。结论:职业紧张与冠脉病变的严重程度呈正相关,外在付出、ERI值和内在投入得分高低影响冠脉病变严重程度,提示护理工作者可根据职业紧张识别严重冠脉病变的高危人群,并进行针对性的护理及随访。

Abstract: Objective: To investigate the association between job stress and the severity of coronary artery disease in young and middle-aged patients with Coronary Heart Disease (CHD). Methods: A total of 419 young and middle-aged patients who was performed coronary angiography in a hospital in Beijing from January to December 2020 were included. They were divided into CHD group and control group by convenient sampling method. Gensini score was used to evaluate the severity of coronary artery disease. The general information questionnaire and the Effort Reward Imbalance Questionnaire (ERIQ) were performed. Spearman correlation and logistic regression was used to analyze the association between job stress and severity of coronary artery disease. Results: There were statistically significant differences in effort, ERI and overcommitment among different coronary artery disease groups (P<0.05). Effort, ERI and overcommitment were positively correlated with Gensini score. Effort, ERI and overcommitment were the independent risk factors for the severity of coronary artery disease, and ERI had the greatest effect. Conclusion: Job stress is positively correlated with the severity of coronary artery disease. The effort, ERI and overcommitment can predict the severity of coronary artery disease.
