新冠肺炎抗疫一线护士创伤后成长现状及影响因素分析

Posttraumatic Growth and its related factors among nursing staff working against COVID-19 in the frontline clinical departments

摘要: 目的:调查新冠肺炎抗疫一线护理人员创伤后成长现状并分析其影响因素。方法:采取便利抽样方法,选取2020年2—4月在武汉市某重型、危重型新冠肺炎患者定点救治医院发热门诊及隔离病区工作的512例护士为研究对象,采用一般资料调查表、中文版创伤后成长量表进行在线调查。结果:新冠肺炎抗疫一线护士创伤后成长总分为(64.35±7.24)分。多元线性回归分析结果显示,职称、文化程度、原工作科室、防疫一线工作时间、是否接受过系统灾害护理培训、有无类似救援经历(如SARS等)、有无亲友/同事确诊感染、护患关系满意度、有无参与危重患者抢救经历是创伤后成长水平的影响因素(P<0.05),共解释创伤后成长42.4%的变异量。结论:新冠肺炎抗疫一线护士创伤后成长处于中等水平,医疗机构应将心理危机干预纳入疫情防控整体部署,指导其采取积极的应对方式,促进正向心理特质的形成。

Abstract: Objective: To investigate the Posttraumatic Growth (PTG) and its related factors among nursing staff working against COVID-19 in the frontline clinical departments. Methods: By convenient sampling, totally 512 nursing staff working in fever clinics and isolation wards of the COVID-19 were included between February, 2020 to April 2020 in a pointed treatment COVID-19 hospital in Wuhan. A general information questionnaire, and Posttraumatic Growth Inventory (PTGI) were used for online data collection. Results: The total score of PTGI was 64.35±7.24 among nursing staff. Multiple regression analysis showed that the related factors of PTGI included professional titles, education, original working departments, time spending on frontline clinical departments against COVID-19, whether received the training in disaster events, whether with similar working experience in some acute incidence (e.g. SARS), whether had relatives or colleagues being diagnosed with COVID-19, nurse-patient relationship satisfaction, whether with experience in the rescue of critically ill patients (P<0.05), and a total of 42.4% variation were explained. Conclusion: The PTGI among nursing staff in the frontline clinical departments were at a moderate level. It's suggested that psychological crisis intervention for medical staff should be included in the management system of epidemic prevention and control, to guide them to adopt positive coping style and establish the development of positive psychological characteristics.
