北京市6所三级医院高年资护士职业高原现状及影响因素分析

Analysis on the current situation and influencing factors of career plateau for senior nurses in six tertiary hospitals in Beijing

摘要: 目的:描述高年资护士职业高原现状并分析其影响因素。方法:于2019年9—12月,采用便利抽样法,以职业高原测量问卷和一般资料调查表对北京市6所三级医院工龄≥5年的护士进行调查,分析其职业高原现状及影响因素。结果:共收集有效问卷2?680份,职业高原总体发生率为34.0%,其中层级高原发生率最高(53.8%),中心化高原发生率为36.7%,内容高原发生率最低(15.6%)。Logistic回归分析结果显示,年龄、用工性质、科室、职称、职务以及是否为专科护士、生活满意度对职业高原有影响。结论:北京市6所三级医院高年资护士约三分之一存在职业高原,对职业高原影响因素的分析为多维度采取应对策略提供了依据。针对不同用工性质及科室的护士从晋升、专业发展、提高生活满意度等方面做好护士职业生涯管理,有助于减少护士职业高原的发生。

Abstract: Objective: To describe the current status of career plateau for senior nurses and analyze its influencing factors. Methods: The convenience sampling method was used and nurses with working age≥5 years in 6 tertiary hospitals in Beijing were recruited from September to December 2019 and administered general questionnaire and career plateau survey questionnaire. Results: A total of 2,680 questionnaires were collected. The overall incidence rate of nurse career plateau was 34.0%, the highest incidence rate of hierarchical plateau was 53.8%, the incidence rate of centralized plateau dimension was 36.7%, and the lowest incidence rate was 15.6% on the content plateau dimension. Logistic regression results showed that age, employment nature, working department, professional title, position, whether she/he was a specialist nurse, life satisfaction were the influencing factors of the career plateau. Conclusion: About one third of senior nurses in six tertiary hospitals in Beijing have career plateau. This study provides the basis for multi-dimensional coping strategies, helps to do the nurses' career management in terms of promotion, professional development and life satisfaction according to nurses of different employment nature and departments, and helps to reduce the incidence rate of nurse career plateau.
