肿瘤患者中心静脉导管相关性感染危险因素的Meta分析

Risk factors for central venous catheter-related infections in cancer patients: a Meta-analysis

摘要: 目的:分析肿瘤患者导管相关性感染的危险因素。方法:检索PubMed、Cochrane library、Web of Science、EBSCO、CBM、CNKI、万方数据库中关于肿瘤患者导管相关性感染危险因素的文章,对结局指标合并后进行Meta分析,敏感性分析和发表偏倚的检测。结果:纳入的19篇文献中,共11?672例患者,957例患者出现感染,发生率为8.2%。涉及到了26项危险因素,除去仅有1篇文献提及的危险因素,对剩余的14项危险因素结果进行分析。年龄≥60岁、有合并症、血液系统肿瘤、穿刺次数≥2次、留置时间≥7个月、右侧手臂、导管调整、化疗次数≥6次、免疫功能低下、输注肠外营养、夏季置管与中心静脉导管相关性感染合并的OR值有统计学意义(P<0.05)。结论:肿瘤患者导管相关性感染受多种危险因素的影响,规范严格的护理操作技术以及降低导管相关性危险因素的影响对预防导管相关性感染至关重要。

Abstract: Objective: To investigate the risk factors for catheter-related infection in cancer patients. Methods: Articles on risk factors for catheter-related infection in cancer patients were retrieved from PubMed, Cochrane Library, Web of Science, EBSCO, CBM, CNKI and Wanfang databases. A Meta-analysis was performed after the combination of outcome indicators. Sensitivity analysis and publication bias was checked. Results: In the 19 articles, a total of 11,672 patients were included, 957 patients were infected, with an incidence rate of 8.2%. Twenty-six risk factors were involved. Excluding the risk factors mentioned in only one article, the remaining 14 risk factors were analyzed. Age (>60 years), comorbidities, hematological tumor, puncture times (>2 times), indwelling time (>7 months), right arm, catheter adjustment, chemotherapy (>6 times), low immune function, TPN infusion, summer catheterization associated with central venous catheter-related infections were statistically significant (P<0.05). Conclusion: Catheter-related infection in cancer patients is affected by multiple factors, and it is important to regulate nursing techniques to reduce the risk and prevent catheter-related infection.
