关节置换术患者缩短术前禁食水时间的可行性研究

Feasibility study on shortening preoperative fasting time in patients undergoing arthroplasty

摘要: 目的:分析关节置换术患者缩短术前禁食水时间的安全性和可行性。方法:选取北京市某三级甲等医院骨关节科的107例接受关节置换术的患者,按时间先后分为对照组(55例)和实验组(52例)。对照组按照传统术前禁食水方法给予指导;实验组术前1天晚12点后禁食,术前2?h前口服完330?mL果糖饮品,之后禁饮。记录并比较两组患者术前1?h、术后1?h和6?h的客观指标(血压、血糖、心率、排气时间、排尿时间等)、主观舒适度(术前焦虑、口渴、饥饿、恶心呕吐),并观察患者不良反应的发生情况。结果:两组患者术后血压、心率、血糖、饥饿程度、排尿时长、恶心呕吐发生情况方面差异无统计学意义(P>0.05)。实验组患者术前焦虑程度、术后口渴程度、排气时间方面优于对照组,且差异有统计学意义(P<0.05)。两组患者均无严重不良反应发生。结论:缩短关节置换患者术前禁食水时间安全、可行,且有助于减轻患者术前焦虑、口渴症状,缩短其术后排气时间。

Abstract: Objective: To evaluate the safety and feasibility of shortening preoperative fasting time in patients undergoing arthroplasty. Methods: A total of 107 patients who underwent arthroplasty from a hospital in Beijing were divided into control group (n=55) and experimental group (n=52) according to the time sequence. The control group was given guidance according to the traditional preoperative fasting method. The experimental group was fasted after 12:00 pm. on the 1st day before surgery, and 330 mL of Fructose drinks were orally administered 2 hours before the operation, and then abstained from drinking and eating. Objective measures (blood pressure, blood glucose, heart rate, exhaust time, urination time, etc) and subjective comfort (preoperative anxiety, thirst, hunger, nausea and vomiting) were recorded and compared between the two groups. And observe the occurrence of adverse reactions in patients. Results: There were no significant differences in postoperative blood pressure, heart rate, blood glucose, hunger, uresis duration, and nausea and vomiting between the two groups (P>0.05). The patients in the experimental group were better than the control group in terms of preoperative anxiety, postoperative thirst and exhaust time, and the difference was statistically significant (P<0.05). There were no serious adverse reactions occurred in either groups. Conclusion: It is safe and feasible to shorten the fasting time before arthroplasty, and it is helpful to reduce the symptoms of preoperative anxiety and thirst and shorten the postoperative exhaust time.
