Evidence summary of airway management for tracheostomy patients during the hospital-to-home transition

Abstract: Objective: To retrieve, evaluate and synthesize evidence on airway management for tracheostomy patients during the hospital-to-home transition, in order to provide a basis for clinical practice. Methods: Based on the "5S" evidence hierarchy model, a systematic search was conducted in Chinese and English databases for literature on tracheostomy airway management from their inception to August 1, 2025. Researchers independently performed quality assessment, evidence extraction, and evidence synthesis. Results: A total of 17 articles were included: 3 clinical decisions, 1 guideline, 1 evidence summary, 4 expert consensus, 1 standard, 4 systematic reviews, 2 randomized controlled trials and 1 cohort study. The evidence was synthesized into 6 themes: transitional care implementation assurance, predischarge assessment and preparation, airway management training and education, home maintenance and management, emergency response and external support, and transitional follow-up, covering a total of 31 pieces of evidence. Conclusion: The evidence is scientifically sound and practically applicable, providing a basis for airway management practice during the hospital-to-home transition.
