Facilitators and barriers to implementing person-centered care among patients with dementia in long-term care facilities

Abstract: Objective: To explore facilitators and barriers to implementing person-centered care among patients with moderate to severe dementia in long-term care facilities. Methods: It is a descriptive qualitative study. We conducted semi-structured interviews with 11 caregivers who participated in the person-centered care in two long-term care facilities. Content analysis method was used to analyze the transcript. Results: Three major themes emerged for facilitators, which were factors related to care plans, organization and implementation, and external support. Two major themes emerged for barriers, which were patient factors as well as factors related to organization and implementation. Conclusion: Developing care plans which are adapted to patients through discussions of multidisciplinary team, arranging caregivers who are acquaintant with patients to help them take part in group activities and having the joined support from managers, caregivers, and family members, are facilitators to implementing person-centered care.
