Construction and application of Long Term Domiciliary Oxygen Therapy management plan for COPD patients based on Knowledge To Action theory

Abstract: Objective: To establish and evaluate the effect of the Long Term Domiciliary Oxygen Therapy (LTDOT)management program for Chronic Obstructive Pulmonary Disease (COPD) patients in a trinity model including hospital, community and family. Methods: Firstly, a comprehensive search of relevant clinical practice guidelines on LTDOT management plan for COPD patients at home and abroad was conducted with the Knowledge To Action (KTA) Framework as the theoretical guidance. The results were then analyzed and combined with clinical practice, upon which the trinity LTDOT management plan for COPD patients was established. A total of 64 COPD patients admitted from early July 2020 to the end of December 2020 were selected as the research objects who were divided into the intervention group and the control group using random number method. The control group was given routine care while the intervention group received the hospital-community-family model of LTDOT care management plan for COPD patients aside from routine care to evaluate and compare their knowledge and compliance of LTDOT, quality of life, oxygen therapy duration, hospitalization expenses, etc. Results: The LTDOT knowledge score, compliance score, oxygen therapy duration and quality of life scores of the intervention group were higher than those of the control group with the difference being statistically significant (P<0.05). Conclusion: The trinity model significantly prolongs the patient's LTDOT time, improves their knowledge, compliance and life quality.
