Abstract: We described the development of home-based hospice care model and introduced the content, methods and management of this model, the composition of core team personnel and the training of team members. We need to promote the development of home-based hospice care from four aspects: top-level design and standards of hospice care, fully utilizing the service capacity of community health centers, introducing social organizations to participate, and improving the support levels for family members and patients.
