老年脑卒中患者自我调节疲劳的体验

A qualitative study on self-regulated fatigue experience of older stroke patients during self-management

摘要: 目的:深入了解老年脑卒中患者自我管理过程中自我调节疲劳的真实体验,为临床护理实践提供参考。方法:运用现象学研究方法,采用目的抽样法,于2024年3月—5月对16名老年脑卒中患者进行半结构式访谈,采用Nvivo?12.0软件和Colaizzi?7步分析法整理分析访谈资料。结果:根据访谈内容共提炼出自我调节疲劳表现过程、急性期自我调节疲劳源、恢复期自我调节疲劳源、后遗症期自我调节疲劳源4个主题和12个亚主题。结论:老年脑卒中患者自我调节疲劳表现具有多维性且程度逐渐增加,在疾病急性期、恢复期、后遗症期自我调节疲劳源存在差异,医护人员应动态关注患者各阶段自我调节疲劳现状,采取针对性措施,维持患者心理资源平衡,提高其自我管理能力。

Abstract: Objective: To gain a deeper understanding of the true experience of self-regulated fatigue in older stroke patients during self-management. Methods: Using phenomenological research methods and purposive sampling, semi-structured interviews were conducted on 16 elderly stroke patients from March to May 2024. Nvivo 12.0 software and Colaizzi seven step analysis method were used to organize and analyze the interview data. Results: Four themes and 12 sub-themes were extracted, including self-regulated fatigue performance process, acute phase self-regulated fatigue sources, recovery phase self-regulated fatigue sources, and sequelae phase self-regulated fatigue sources. Conclusion: The manifestation of self-regulated fatigue in older stroke patients is multidimensional and gradually increases in severity. There are differences in the sources of self-regulated fatigue in the acute, recovery, and sequelae stages of the disease. Medical staff should dynamically pay attention to the status of self-regulated fatigue in older stroke patients at each stage, take targeted measures, maintain the balance of patients' psychological resources, and improve their self-management ability.
