高磷血症透析患者控磷饮食依从性现况与影响因素分析

The compliance with phosphorus-controlled diets and its influencing factors of Hemodialysis patients with hyperphosphatemia

摘要: 目的:调查高磷血症血液透析患者控磷饮食依从性现况并分析控磷饮食依从性的影响因素。方法:采用便利抽样法,选取2018年5月至2019年10月在广西某两所三级甲等医院进行血液透析的172例高磷血症患者作为研究对象,使用一般资料调查表、高磷血症血液透析患者饮食依从性问卷、血液透析高磷血症患者低磷饮食健康信念量表对其进行调查。结果:高磷血症血液透析患者饮食依从性得分为(36.39±3.55)分,总体依从率为60.6%,处于中等水平;多元线性回归分析显示,性别、受教育程度、职业状态、饮食习惯、低磷饮食健康信念是高磷血症血液透析患者饮食依从性的影响因素(P<0.05)。结论:高磷血症血液透析患者控磷饮食依从性有待提高,在临床工作中需要加强对男性、在职、受教育程度低、饮食习惯差、低磷饮食健康信念弱的患者进行关注,实施行之有效的措施,提高患者控磷饮食的依从性。

Abstract: Objective: To investigate the compliance with phosphorus-controlled diets and its influencing factors in hemo-dialysis patients with hyperphosphatemia. Methods: A convenience sampling was used to recruit 172 hyperphosphatemic patients who underwent hemodialysis in two tertiary grade A hospitals in Guangxi from May 2018 to October 2019. Patients were investigated by a general information questionnaire, a dietary compliance questionnaire for hyperphosphatemic hemo- dialysis patients, and a low phosphorus diet health belief scale in hemodialysis patients with hyperphosphatemia. Results: The total score of dietary compliance of hyperphosphatemic patients was (36.39±3.55) and the overall compliance rate was 60.6%, which were at the moderate level. Multiple linear regression analysis showed that gender, education level, occupational status, dietary habits, and health beliefs about the low phosphorus diet were the influential factors of dietary compliance of hyperphosphatemic patients (P<0.05). Conclusion: The compliance of hyperphosphatemic Hemodialysis patients with phosphorus-controlled diets needs to be improved. In clinical settings, we need to pay more attention to patients who are male, employed, poorly educated, with poor dietary habits, and with weak believes in low phosphorus diet. Tailored and effective measures are suggested to these patients to improve their compliance with phosphorus-controlled diets.
