心脏术后危重患者家属需求调查及影响因素研究

The needs of family members of critical patients with heart disease after operation

摘要: 目的:了解心脏术后危重患者家属需求的现状和影响因素,探讨家属需求与焦虑、抑郁之间的相关性。方法:2018年1月至12月采用方便抽样法选取湖南省某三级甲等医院心血管外科重症监护室196例心脏术后危重患者的家属为研究对象,采用一般资料调查表、危重患者家属需求量表、焦虑自评量表、抑郁自评量表进行问卷调查。结果:心脏术后危重患者家属需求从高到低分别为病情保证(3.45±0.56)分、获取信息(3.32±0.57)分、亲近患者的需求(3.20±0.57)分、医护人员的支持(2.74±0.58)分和家属自身的舒适(2.59±0.73)分。家属认为最重要的需求是“保证患者得到最佳的救治”,最不重要的需求是“希望能允许有时大声哭叫”。不同年龄组、文化程度、患者疾病诊断、患者与家属的关系之间家属的需求侧重不一样。其中医护人员的支持、获取信息、病情保证与家属的焦虑、抑郁水平呈正相关。结论:心脏术后危重患者家属最需要得到满足的需求是病情保证,其次是获取信息、亲近患者,这为心外ICU患者及其家属提供更好的护理和支持提供了临床指导,以期促进有效沟通、建立良好的医患联系。未来可通过满足ICU患者家属需求,从而降低其焦虑、抑郁状态。

Abstract: Objectives: To investigate the needs of family members of critical patients with heart disease after operation, and to analyze the relationships among needs, anxiety and depression in the family members of critical patients with heart disease after operation. Methods: A total of 196 family members of critical patients with heart disease after operation were recruited by convenience sampling from the Cardiac Intensive Care Unit in a tertiary hospital of Hunan province from January to December 2018. Each family member was asked to complete the General Status Questionnaire, Critical Care Family Needs Inventory (CCFNI), Self-rating Anxiety Scale (SAS), and Self-rating Depression Scale (SDS). Results: The most important need identified by the family members was the assurance of patient's condition (3.45±0.56), followed by information acquisition (3.32±0.57), being close to patients (3.20±0.57), supports from healthcare professionals (2.74±0.58), and their own comfortable feeling (2.59±0.73). "Ensuring the best treatment for patients" is on the top list, while "hoping the permission to cry out loudly sometimes" is on the bottom of the list. There are differences in needs of family members with different age, level of education, disease diagnosis, and family relationship. The supports of healthcare professionals, information acquisition, and assurance of patient's conditions were positively correlated with the anxiety and depression level of these family members. Conclusion: The most important need is assurance of patient's condition, and then the information acquisition and being close to the patients. This finding provides references for the health professionals to provide support and care to patients and their family members. Thus we can promote effective communication and build up good relationship between health professionals and patients. In the future, the anxiety and depression level of family members can be decreased by meeting their needs.
