肺癌患者营养管理方案的构建及应用

Construction and application of the nutritional management program for lung cancer patients

摘要: 目的:评价基于E-Coach健康管理模式的营养管理方案对肺癌患者营养的护理效果,为规范、科学管理肺癌患者营养提供参考依据。方法:便利抽样选取2023年6月至2024年6月长春市某三级甲等医院收治的78例肺癌化疗患者,用计算机随机数字法分为对照组和干预组,每组39例。对照组实施常规营养管理,干预组采用基于E-Coach健康管理模式的营养管理方案。在入组当日(T0)、干预措施全部完成之时(T1)及干预结束3个月后(T2),调查两组患者营养状况、生活质量、身体活动能力及营养管理依从性。结果:在T1时,干预组营养不良发生率为56.4%,显著低于对照组的94.8%,差异具有统计学意义(χ2=15.656,P<0.001);两组前白蛋白、血清白蛋白、血红蛋白、生活质量、身体活动能力和营养管理依从性差异具有统计学意义(P<0.05)。两组在T2时的营养状况(除肌肉量水平)、生活质量、身体活动能力和营养管理依从性,差异有统计学意义(P<0.01)。结论:构建的营养管理方案能有效改善肺癌患者营养状况,提升生活质量与身体活动能力,具有临床推荐价值。

Abstract: Objective: To explore the effects of the E-Coach health management model on nutritional status of patients with lung cancer, so as to provide a reference for standardized and scientific management of nutrition in lung cancer patients. Methods: A total of 78 lung cancer patients admitted to a tertiary hospital in Jilin province from June 2023 to June 2024 were recruited and were randomly assigned to the control group and observational group evenly. Patients in the control group received routine care and patients in the observational group received care based on the E-Coach health management model. The nutritional status, quality of life, physical activity and compliance with the nutrition management of the two groups were collected on the day of enrollment (T0), intervention completion (T1), and 3 months after intervention completion (T2). Results: At T1, the incidence of malnutrition in the intervention group was 56.4%, lower than the 94.8% in the control group, and the difference was statistically significant (χ2=15.656, P<0.001); Significant differences were also observed regarding prealbumin, albumin and hemoglobin levels, as well as quality of life, physical activities, and compliance with the nutrition management between two groups (P<0.05). Participants' nutritional status (except for muscle mass), quality of life, physical activities, and compliance with the nutrition management at T2 were statistically different between two groups (P<0.01). Conclusion: The nutritional management program can effectively improve the nutritional status, quality of life, and physical activity ability of lung cancer patients. The intervention is worthy of promotion in clinical practice.
