Abstract: Objective: To understand the real experience of disease management at home of elderly patients with chronic heart failure to provide reference for intervention and management. Methods: Using descriptive phenomenological research approaches, semi-structured interviews were conducted in 22 elderly patients with chronic heart failure, and information was analyzed using the Colaizzi 7-step analysis. Results: Four themes were summarized based on the interviews: heavy burden of disease management; insecurity as an important psychological burden; family and caregivers are spiritual support; different coping attitudes. Conclusion: A proper understanding of the challenges faced by elderly patients with chronic heart failure in the process of disease management at home, focusing on patients' experiences, and exploring targeted intervention should be the focus of future intervention studies.
