Abstract: The paper is to introduce relevant concepts of transition from children to adult clinics for patients with Inflammatory Bowel Disease. It is also to summarize the existing transition models, analyze factors affecting successful transition, and point out the strategies for successful transition, so as to provide guidance and help for patients with Inflammatory Bowel Disease who transit from the children clinic to adult clinic.
