远程康复模式应用于脑瘫患者临床效果的Meta分析

Meta-analysis of the effects of telerehabilitation in patients with Cerebral Palsy

摘要: 目的:评价远程康复模式在脑瘫患者中的应用效果。方法:计算机检索Cochrane Library、PubMed、Web of Science、中国期刊全文数据库(CNKI)、万方数据库(Wanfang Data)和维普数据库(VIP)等数据库,收集有关远程康复模式应用于脑瘫患者的随机对照试验。根据纳入和排除标准,对文献进行筛选,提取数据并进行文献质量评价,采用RevMan?5.3软件进行Meta分析。结果:最终纳入9篇随机对照试验,共计281例患者。Meta分析结果显示,远程康复组与传统康复组相比,脑瘫患者的站立功能以及步行、跑步和跳跃功能显著改善,肢体灵敏度增加,差异有统计学意义(P<0.05);在提高患者的步行速度以及降低下肢肌张力方面差异无统计学意义(P>0.05);以儿童平衡量表为评价指标时,脑瘫患者的平衡功能显著改善,差异有统计学意义(P<0.05),以起立-步行测试为评价指标时,在脑瘫患儿的平衡功能改善方面差异无统计学意义(P>0.05)。结论:远程康复模式可显著改善脑瘫患者的运动功能和肢体灵敏度,但在提高脑瘫患者步行速度和降低下肢肌张力方面无明显作用,对促进其平衡功能方面尚未有定论,须开展深入研究做进一步验证。

Abstract: Objective: To evaluate the effects of telerehabilitation in patients with Cerebral Palsy through Meta-analysis. Methods: Database such as Cochrane Library, PubMed, Web of Science, CNKI, VIP and Wanfang were searched to retrieve Randomized Controlled Trials (RCT) on telerehabilitation for Cerebral Palsy. The Meta-analysis was conducted by RevMan 5.3 after data extraction and quality appraisal of the eligible RCTs. Results: A total of 9 RCTs were included in this study, with a total of 281 subjects.The results of Meta-analysis supported the effectiveness of telerehabilitation on improving the function of standing, walking, running and jumping, and increasing the limb sensitivity of the patients with Cerebral Palsy (P<0.05). It failed to support the effectiveness of improvement of walking speed and reduction of the muscle tone of lower limb (P>0.05). When the Pediatric Balance Scale (PBS) was used as the evaluation criteria, the balance function of patients with Cerebral Palsy could be significantly improved (P<0.05). When the Timed-Up and Go Test (TUGT) was used as the evaluation criteria, there was no significant effect on the balance function of children with Cerebral Palsy (P>0.05). Conclusion: Telerehabilitation can significantly improve the motor function and increased the limb sensitivity of patients with Cerebral Palsy. However, it failed to enhance the effect of improving walking speed and reducing the muscle tone of lower limbs on patients with Cerebral Palsy. There is no conclusion on promoting the balance function of patients with Cerebral Palsy, which needs further research to verify.
