Abstract: This paper introduces the practice and exploration in Changzheng Zhen Community Healthcare Center of Putuo district of Shanghai during the national level community hospice care pilot work, supported by city and district policies, from the aspects of hardware construction, team building, system flow, service content, publicity and education, quality control, etc. So as to form a community hospice service mode combining home and hospitalization, and develop thinking and outlook for this case.
