武汉市三级甲等医院护士离职意愿现状及影响因素分析

Current status and influencing factors of clinical nurses' turnover intention in tertiary grade A hospitals in Wuhan

摘要: 目的:调查武汉市三级甲等医院护士离职意愿现状,并分析其影响因素,为制定相关措施降低护士离职意愿提供科学依据。方法:采用整群抽样方法抽取武汉市6所三级甲等医院1?667名护士进行问卷调查,调查内容包括一般人口学特征、护士工作压力、共情满意、离职意愿等。采用有序多分类Logistic回归分析离职意愿的影响因素。结果:47.3%的护士近半年有离职意愿,其中21.7%一直有离职的意愿;26~35岁的护士较其他年龄护士更可能有离职意愿;相对于硕士及以上学历,大专和本科学历的护士离职意愿更强烈;外科、妇产科、儿科护士较其他科室更容易产生离职意愿;高水平工作压力、低水平共情满意者有较强的离职意愿。结论:武汉市三级甲等医院护士离职意愿较高,尤其是大专和本科学历,以及在外科、妇产科和儿科工作的护士,工作压力水平、共情满意程度与离职意愿有关。结合内在、外在因素,管理者应关注护士的工作压力和共情满意,从而降低护士的离职意愿。

Abstract: Objective: To investigate the prevalence of turnover invention and to explore its influencing factors among nurses in tertiary grade A hospitals in Wuhan, and to provide evidence for targeted intervention to decrease nurse turnover. Methods: A clustered sample of 1 667 nurses were recruited from 6 tertiary grade A hospitals in Wuhan and completed the Chinese Nurse Job Stressors Questionnaire, Compassion Satisfaction Questionnaire and intention to leave profession. Multivariate logistic regression was used to analyze main influencing factors of turnover intention. Results: About 47.3% of the nurses intended to leave their profession in the past six months. Nurses aged 26 to 35 are more likely to leave their jobs than nurses of other ages. Compared with nurses with master's degree or above, associate college and bachelor nurses are more likely to have a leaving intention. Surgical, gynecology, pediatric nurses are more likely to have a willingness to leave than nurses in other units. Nurses with high level job stress and low compassion satisfaction were prone to have the intention to leave the profession. Conclusion: Nurses in Wuhan have a higher turnover invention, specially nurses with associate college and undergraduate degree, working in surgery, gynecology and pediatrics departments. Job stress and compassion satisfaction are significantly related to turnover intention. Nursing managers should pay more attention to internal and external characteristics, job stress and compassion satisfaction among nurses to decrease nurse' turnover intention.
