Oladayo Bifarin, Emily Oliver, Jan Oyebode, LIU Yu
Chinese Nursing Management. 2020, 20 (9):  1285-1291.  DOI: 10.3969/j.issn.1672-1756.2020.09.002
Abstract ( 361 )   PDF (4334KB) ( 250 )
Through this article we aim to contribute to discussion about how to support family caregivers in China to care effectively for parents with dementia. We outline evidence about the nature of the challenge, situating the rise in numbers of people living with dementia and the role of families in providing care, in the specific time and place of 21st century China. We note the unique backdrop of economic and social transitions, which is leading to strains and changes in traditional values. We draw on recent research to summarise the impact of caregiving and suggest the socio-cultural model of stress and coping as a powerful framework to inform service development. Given the centrality of filial piety in Chinese culture and the associated expectation that families look after their own relatives, we propose that a community case management model may be an effective way of supporting dementia care in China. We describe the Admiral Nursing model that is used in the UK as an example.

Objective: To compare the incidence rate and influencing factors of falls between elderly and middle-young maintenance hemodialysis patients. Methods: We performed a multicenter cross-sectional interview-based survey to determine one-year fall incidence rate and fall characteristics in 1230 dialysis patients from thirteen public hospitals in Jiangsu Province by using convenience sampling. Results: Among the 1230 patients, there were 349 patients aged 65 years or above, and 881 patients aged under 65 years of age. The fall incidence rate among elderly maintenance dialysis patients and middle-young patients were 23.2% and 10.7%, respectively. Whether having poor balance and gait, secondary hyperparathyroidism and poor eyesight or hearing were identified as independent fall-related factors among elderly maintenance dialysis patients. Duration of dialysis, whether having secondary hyperparathyroidism, chronic pain, intradialytic hypotension, whether using auxiliary equipment were identified as independent fall-related factors among middle-young maintenance dialysis patients. Conclusion: The fall incidence rate among elderly dialysis patients is much higher than that of middle-young patients. There are more independent fall-related factors in middle-young maintenance hemodialysis patients and they are more closely related to dialysis. The shared independent fall-related factor between two groups is whether having secondary hyperparathyroidism.

Abstract ( 283 )   PDF (3230KB) ( 225 )
Objective: To design and apply a Prone Position Ventilation (PPV) implementation checklist for patients with Acute Respiratory Distress Syndrome (ARDS). Methods: Based on literature review and clinical simulation, an implementation checklist for PPV was developed for patients with ARDS, which was applied to patients who required PPV in ICU. Following a pretest and posttest method, the rate of complications and the duration of PPV were compared before and after the application of the checklist. Results: After implementation of the PPV checklist, catheter slippage rate decreased from 52.9% to 10.5%, pressure injury rate decreased from 41.2% to 5.3% in patients with ARDS, and the average duration of prone position ventilation increased. Conclusion: Application of PPV checklist in patients with ARDS requiring PPV can effectively reduce the incidence of complications associated with ventilation, promote PPV, prolong average duration on ventilation and increase the effect of ventilation in prone position. This study demonstrated that the PPV checklist we developed is beneficial in standardizing the operational procedures and ensuring consistency in implementation management.
