Construction and practice of evidence-based management plan for fall prevention in hospitalized patients

Objective: To develop an evidence-based fall prevention management protocol for hospitalized patients based on the Comprehensive Unit-based Safety Program and evaluate its clinical effectiveness, and to provide references for improving the systematic management of falls in hospitalized patients. Methods: Based on literature review and expert consultation, an evidence-informed fall management protocol was developed. A stepped-wedge cluster randomized trial design with purposive sampling was employed. Nursing units with fall incidence rate higher than the hospital-wide average from 2020 to 2022 and their nurses were selected as participants. Intervention began in June 2023, the pre-intervention phases followed routine nursing practices with reference to the hospital-wide unified fall management documents, while the post-intervention phases implemented the evidence-informed fall management protocol. Outcomes included fall incidence, injury rates, and nurses' safety attitudes and safety culture cognition levels. Results: Eight nursing units and 158 nurses were included. After the intervention, the incidence (0.49‰) and injury rate (25.00%) of falls were lower than before (1.06‰ and 68.18%), nurses' safety attitude and safety culture cognition scores were higher than before, the differences were all statistically significant (P<0.05). Conclusion: The developed evidence-informed fall management protocol exhibits scientific rigor and practical applicability, reduces the incidence of falls and injury rate, and promotes nurses' safety attitude as well as safety culture cognition.

Objective: To sinicize The Nursing Presence Scale for Hospitalized Patients and test its reliability and validity, so as to provide a reliable and effective evaluation tool for clinical investigation of patients' nursing experience. Methods: After obtaining authorization from the original author of the scale, we translated the English version of The Nursing Presence Scale for Hospitalized Patients and conducted back-translated, and carried out cultural adaptation. Formal questionnaire survey were conducted among the 530 inpatients of a tertiary grade A hospital in Wuhan by convenience sampling. Results: The Chinese version scale contains 33 items and was divided into 4 dimensions. The Cronbach's α for the total scale was 0.931, and for the dimensions ranged from 0.738 to 0.852. The content validity index of the scale was 0.98. Confirmatory Factor Analysis (CFA) showed that χ2/df was 1.897, GFI was 0.812, CFI was 0.869, IFI was 0.871, TLI was 0.859, RMR was 0.024, RMSEA was 0.062, which indicated that the four-factor model provided the best fit. Conclusion: The Chinese version of The Nursing Presence Scale for Hospitalized Patients has good reliability and validity, and is suitable for evaluating Chinese inpatients' feelings and experience of nursing presence.

Accordingly, the level of kinesiophobia in patients will be decreased, the exercise behavior will be promoted, and the quality of life and prognosis of patients with Chronic Obstructive Pulmonary Disease will be improved finally.

Objective: To translate the Chronic Illness Anticipated Stigma Scale (CIASS) into Chinese, and to test the reliability and validity among patients with permanent enterostomy, in order to provide a measuring tool for the evaluation of anticipated stigma among patients with permanent enterostomy in China. Methods: The Brislin model was adopted to conduct translation and cultural adjustment, pre-investigation of the CIASS was conducted. From March to December 2023, 541 patients with permanent enterostomy in three tertiary grade A hospitals in Hunan province were investigated by using Chinese version of CIASS (CCIASS) to test the reliability and validity of the scale. Results: The I-CVI of the scale was 0.853-1.000, the S-CVI/UA was 0.865, and the S-CVI/Ave was 0.973. The KMO value was 0.893, and Bartlett's sphericity test chi square value was 1128.531 (P<0.01). Three common factors were extracted, and the cumulative variance contribution rate was 68.725%. The results of confirmatory factor analysis showed that the model fitted well, with χ2/df of 2.027, RMSEA of 0.073, GFI of 0.915, CFI of 0.918, IFI of 0.936 and TLI of 0.922. In addition, the total score of CCIASS is positively correlated with the total score of Social Impact Scale (r=0.655, P<0.01). The total Cronbach's α coefficient of CCIASS was 0.925, Cronbach's α coefficients of friends and family members, work colleagues, and healthcare workers dimension were 0.907, 0.893 and 0.916 respectively, and the test-retest reliability was 0.872. Conclusion: The CCIASS contains 3 dimensions and 12 items, and its reliability and validity are good in patients with permanent enterostomy, so it can be used to evaluate the anticipated stigma of patients with permanent enterostomy.

This article reviews the definition and evolution of patient journey mapping and related definitions, common presentation formats and construction methods, as well as the progress in their application to improve patient experience. It explores its role in optimizing care processes, improving the care environment, enhancing doctor-patient communication, and assisting special populations. Additionally, it provides outlooks for using patient journey maps to improve nursing services, offering a reference for its application in the field of clinical healthcare.

This paper systematically analyzes the common problems in knowledge translation research in the field of nursing in China, focusing on three key research components: evidence summary, evidence-based nursing practice, and quality improvement, which share common issues including insufficient understanding of research nature, inappropriate application of methodology, and lack of scientific theoretical guidance. The paper proposes several strategies for improvement: clarifying the relationships and definitions among the three components, emphasizing collection and evaluation of diverse evidence types, enhancing scientific rigor in context-oriented intervention selection and evaluation, and incorporating adaptive designs. Future directions suggest transitioning toward an "Evidence-based Nursing+" model, advancing evidence-based nursing practice through conceptual updates, methodological innovations, and practical reforms.

With the advancement of medically assisted reproductive technology, reproductive-age cancer patients are increasingly likely to preserve their fertility before receiving treatment. This study provids an overview of the decision-making motives regarding fertility preservation among reproductive-age cancer patients, along with the current status of research both domestically and internationally, influencing factors, and interventions, and to provide a theoretical basis for clinical staff to promote timely referral for cancer patients with fertility needs and to improve the quality of patients' decision-making.

Objective: To integrate qualitative research on the experience of stroke patients participating in rehabilitation training with Virtual Reality (VR) technology, and to provide a reference basis for clinical development and improvement of rehabilitation training with VR technology. Methods: The Chinese and English databases were searched for qualitative studies on stroke patients' participation in VR rehabilitation training experience, and the search period was from the establishment of the databases to March 2024. The quality of the literature was evaluated according to the Quality Assessment Criteria for Qualitative Research of the JBI Centre for Evidence-Based Health Care in Australia, and the results were integrated using the Pooled Meta-Integration Method. Results: A total of 13 studies were included, and 43 findings were distilled, 11 new categories were generalised, and integrated into 3 integrated outcomes: higher acceptance of VR technology for rehabilitation training in stroke, perceived benefits of participation in VR technology rehabilitation training, and perceived barriers for stroke patients. Conclusion: VR technology brings a positive rehabilitation experience to most stroke patients, but still faces multiple barriers such as cognitive, physical and psychological barriers. Healthcare professionals should alleviate patients' negative emotions to increase acceptance of VR technology rehabilitation training, strengthen patients' perception of the benefits of VR rehabilitation training to enhance motivation and confidence in participation, and reduce the barriers and challenges to participation in VR technology rehabilitation training to optimize the rehabilitation training experience.

Objective: To systematically evaluate the medication management experience of family caregivers of aged patients with polypharmacy to provide a reference for developing individualized support strategies for family caregivers to participate in medication management. Methods: We searched both Chinese and English databases to include eligible literatures whose quality was later evaluated. The retrieval time limit was from the inception to 26th, December 2023. The results were summarized using the meta-synthesis of qualitative research. Results: A total of 11 articles were included from which 39 research topics were extracted and summarized into 13 new categories. The new categories were grouped into 4 integrated findings: the tasks and participation types of family caregivers; influencing factors of family caregivers' participation in medication management; the challenges and needs of family caregivers; the effective strategies adopted by family caregivers in the process of medication management. Conclusion: The experience of family caregivers participating in medication management is complex and diverse. Medical staff should respect the different participation types caregivers in medication management practice and provide professional and individualized guidance for them to improve their participation experience, and provide medication management support services for elderly patients with polypharmacy and their caregivers.

Objective: To construct and evaluate a Traditional Chinese Medicine Nursing Decision Support System (TCMNDSS) based on clinical needs, aiming to provide scientific and objective decision support tools for nursing staff in their practice of Traditional Chinese Medicine (TCM) nursing. Methods: A questionnaire survey was conducted to collect clinical needs of TCM nurses, and an expert discussion meeting was held to determine the TCMNDSS function list. TCMNDSS was developed and put into use. A total of 60 nurses from a tertiary grade A TCM hospital in Jiujiang city from October to December 2023 were recruited as the research participants and randomly divided into a control group (n=30) and an intervention group (n=30). The nurses in the control group used the traditional TCM nursing model, and the intervention group implemented the TCM nursing model assisted by TCMNDSS. The clinical decision-making ability, decision-making quality, decision-making efficiency of the two groups of nurses were compared. The experience of the nurses in the intervention group on TCMNDSS were investigated. Results: TCMNDSS included the following functions: TCM operation taboos, TCM acupoint positioning, TCM food taboos, TCM nursing plan, and TCM health education. The clinical decision-making ability, and decision-making quality of the intervention group were higher than those of the control group (P<0.05). The decision-making efficiency of the five functions of the intervention group was higher than that of the control group (P<0.05). The intervention group scored high on the experience of using TCMNDSS. Conclusion: The construction process of TCMNDSS based on clinical needs is scientific, which can be applied in clinical practice and assist TCM nurses in making decisions, and nurses have a good experience with TCMNDSS.

Objective: To develop a decision support tool for fertility preservation in female of reproductive age with cancer and evaluate its application effects, aiming to provide clinical guidance for fertility preservation in those patients. Methods: This study utilized literature analysis, qualitative interviews, and expert consultations to develop the decision support tool. Fifty female cancer patients of reproductive age admitted to the oncology department of a tertiary grade A hospital in Hunan province from November 2023 to January 2024 were selected by convenient sampling method, and they were randomly divided into the control group and the intervention group using a random number table, with 25 cases in each group. The control group received routine fertility protection decision-making process, while the intervention group utilized the fertility preservation decision support tool. Scores of fertility preservation knowledge questionnaire, decision self-efficacy scale, and decision conflict scale were compared between the two groups. After the intervention, subjects' acceptance and evaluation of the tool were collected. Results: Compared to the control group, patients in the intervention group scored higher on the fertility preservation knowledge questionnaire and the decision self-efficacy scale, and lower on the decision conflict scale, all with statistically significant differences (P<0.05). The user acceptance score for the intervention group was 16.84±1.49, indicating a high level of acceptance for the fertility preservation decision support tool among female cancer patients of reproductive age. Conclusion: Patients show a high level of acceptance for the fertility preservation decision support tool for female cancer patients of reproductive age. This tool can effectively enhance patients' knowledge of fertility preservation and their decision self-efficacy, while reducing decision conflict.

Objective: Based on the patient journey mapping, the multidimensional needs in the health management of young and middle-aged enterostomy patients with colorectal cancer are visualized, providing information support for optimizing the whole-journey care for those patients. Methods: Using descriptive research methods, 13 young and middle-aged enterostomy patients with colorectal cancer were selected from December 2023 to March 2024. Content analysis was employed to analyze the interview data and draw the journey map of the patients. Results: According to the timing of diagnosis and treatment, from the three dimensions of task, emotion and pain points, the health management needs of patients were subdivided into 30 themes, including early screening and early diagnosis, symptom management, excretion concealment, care support, social adjustment and information screening, to establish the journey map of young and middle-aged enterostomy patients with colorectal cancer. Conclusion: The diversity and complexity of the patients's health management needs make their cancer journey long and complicated. In the future, in light with the various health management needs of such patients, it is necessary to study and construct a health management resource matching model with complementary authoritative medical care and high-quality peer support, so as to synergistically support patients' physical and psychosocial needs, accurately provide quality whole-journey care support to colorectal cancer patients with colostomy.

Relationships among social support, self-efficacy, and frailty in disabled older adults and their family caregivers: the actor-partner interdependence analysis

Objective: To examine the relationships among frailty, social support, and self-efficacy in disabled older individuals and their family caregivers with the objective of providing a foundation for the development of empowerment intervention plans tailored for clinical and community workers. Methods: Based on the actor-partner interdependence model, a convenience sample of 402 disabled older adults-caregiver dyads from 10 communities in Shanghai were surveyed using a general information questionnaire, the Tilburg Frailty Indicator, the General Self-Efficacy Scale, and the Social Support Rating Scale. Correlational analyses were conducted and an actor-partner interdependence model was established. Results: The incidence of frailty among the disabled older adults was 92.8%, significantly higher than the 52.5% incidence among caregivers (P<0.05). The social support scores were (32.69±11.55) for the older adults and (35.54±8.42) for the caregivers. The general self-efficacy scores were (23.50±5.63) and (26.14±7.23) for older adults and their caregivers respectively. Regarding the subject effect, both social support and self-efficacy in disabled older adults and their caregivers had negative impacts on their own frailty levels. As for the object effect, the social support level and self efficacy among disabled older adults moderately influenced the caregiver's frailty, while the caregiver's social support and self-efficacy similarly had negative impacts on disabled older adutls. Significant subject-object effects were observed. Conclusion: The study shows bidirectional interactions among social support, self-efficacy, and frailty in disabled older adults and their caregivers, suggesting that disabled older adults and their caregivers should be considered as a whole. The implementation of dual health empowerment is vital to enhance self-efficacy and social support for both parties to decelerate the progression of frailty.

Objective: To design a volume record sheet for patients with heart failure and identify its clinical application effects, and to provide a reference for volume management quality improvement. Methods: The volume record sheet was designed based on literature review, group discussions, and brainstorming. Participants with heart failure were conveniently selected from a tertiary grade A hospital in Sichuan province. The rates of volume records, the standardization of records, the timely treatments of oral potassium, the compliance of recording at home, and the readmission rates in 3 months after discharge were compared before and after the application of the recording sheet. Results: After the application of the volume record sheet, the rates of volume records, the standardization of records, the timely treatments of oral potassium, and the compliance of recording at home in patients with heart failure were increased significantly by 32.7%, 10.1%, 22.1%, and 19.9%, respectively (P<0.05). There was no statistically significant difference in 3-month readmission (P>0.05). Conclusion: The application of volume record sheet in patients with heart failure is helpful to standardize the volume record process, improve the record completeness and standardization, to increase record compliance at home, and further provides a scientific reference for better volume management decision.

Objective: To explore the effects of application of improvement science in reducing the incidence of PICC Catheter Related Bloodstream Infection (CRBSI) based on evidence-based medicine. Methods: In this study, a convenience sampling method was adopted to select children with PICC in the wards of a tertiary grade A children's hospital and nurses in the ward from January 2020 to December 2022. The control group adopted conventional PICC infection prevention and control measures, while the experimental group adopted the six sigma in the improved science strategy. The incidence of CRBSI and the nurses' knowledge in the two groups was analysed. Results: The incidence of PICC CRBSI of the experimental group was significantly lower than that of the control group (0.9% VS. 2.4%). The incidence of infection per thousand PICC of the experimental group was significantly lower than that of the control group (0.41‰ VS. 1.0‰). The nurses' knowledge of infection prevention and infection control of PICC was higher than that of the control group , and the difference was statistically significant (P<0.05). Conclusion: Evidence-based nursing practice based on improvement science can effectively reduce the incidence of PICC CRBSI, improve nurses' knowledge of catheter infection prevention and control, and improve the quality of clinical nursing work.

Results: After applying the decision support system, the correct rate of PI staging judgment increased from 84.5% to 96.0%, and the correct rate of tissue type judgment increased from 79.5% to 93.5%, the effective rate of PI treatment increased from 68.2% to 82.4%, the implementation rate of PI nursing records measured at 4 time points has increased from 75.4%, 81.1%, 88.3%, and 77.7% to 92.7%, 96.0%, 94.8%, and 94.3%, respectively, the differences were statistically significant (P<0.05). Conclusion: The use of the PI nursing decision support system improved the effectiveness of PI treatment and the standardization of nursing records, promoting an increase in the quality of PI nursing care.

The continuous development of large language models based on artificial intelligence technology has brought significant opportunities for nursing practice. This article provides a comprehensive review of the application of large language models in nursing practice and management, nursing research, and nursing education. It points out that the application process faces risks and challenges such as privacy and security issues, risk of health damage, academic and educational integrity, and domestic environmental applicability. It is recommended that developers and users establish multidisciplinary teams in the future to properly address the existing risks and challenges, so as to accelerate the development of the nursing industry in the era of information technology.

Objective: To analyze the clinical application effect of early goal-directed mobilization protocols in patients after pancreatic tumor resection, in order to improve the effectiveness and initiative of early activity and promote postoperative recovery of patients. Methods: We randomly selected 35 patients who underwent pancreatic surgery at a tertiary grade A hospital in Shanghai from March to May 2023 as the control group, and 35 patients from June to August 2023 as the experimental group. The control group was treated with routine nursing measures after pancreatic tumor resection, and the experimental group applied the early goal-directed mobilization protocols in supervising the early activities of the patients. The effectiveness of the protocol was evaluated by comparing the time of first getting out of bed, flatus, defecation, oral drinking, urinary catheter removal and gastric tube removal between the two groups. The occurrence of postoperative complications and activity-related adverse events were compared between the two groups to evaluate the safety of the protocol. Results: There were statistically significant differences in the time of first getting out of bed, flatus, defecation and oral drinking between the two groups (P<0.05). There were no significant differences in urinary catheter removal time, gastric tube removal time and postoperative complications between the two groups (P>0.05). No exercise-related adverse events were observed in either group. Conclusion: Early goal-directed mobilization protocols is safe and effective in promoting early ambulation, early drink oral water and postoperative ventilation and defecation after surgery, no increased risk of postoperative bleeding, unplanned extubation and death.

Exercise intervention on the quality of life in patients with Systemic Lupus Erythematosus: a Meta-analysis

Objective: To evaluate the effect of different exercise programs on the quality of life of patients with Systemic Lupus Erythematosus (SLE). Methods: According to the PICOS principle, an experimental study on the impact of exercise intervention on the quality of life of SLE patients published before November 2023 was retrieved from Chinese and English databases. The study was evaluated by the literature quality evaluation tool published by the Australian JBI Evidence Based Health Center. RevMan 5.4 was applied for data analysis. Results: Nine studies with 529 subjects were included exercise program was classified into aerobic exercise and resistance exercise. Subgroup analysis showed that patients exercised ≥3 times per week had significant improvements in quality of life including physical health [SMD=0.69, 95% CI (0.48, 0.90), P<0.001] and mental health [SMD=0.90, 95% CI (0.45, 1.35), P<0.001]. Patients exercised 30-50 minutes each time showed significant improvements in Quality of Life including physical health [SMD=0.72, 95% CI (0.41, 1.04), P<0.001] and mental health [SMD=0.81, 95% CI (0.36, 1.26), P<0.001]. Conclusion: SLE Patients who exercise ≥3 times a week for about 30-50 minutes each time have a significant improvement in their quality of life. Further large-scale high-quality RCT studies are needed to elucidate the precise exercise plan for patients with SLE.

Objective: To conduct a comprehensive review of the literature on oral care management for patients with stroke, and to provide evidence-based support for the screening and development of oral care management for patients with stroke in China. Methods: The framework of scoping review was used to search the literature on stroke oral care management in 9 databases from the inception to October 13, 2023. Results: A total of 22 studies were included, covering 10 oral symptoms or related complications and a total of 27 outcome indicators. The oral care management included the development of oral intervention tools, the oral organs and surrounding tissues, the intervention measures and the oral assessment. While there was a lack of comprehensive and universal oral care management programs and standardized models for patients with stroke. Conclusion: The oral care management programs for stroke patients varies and are still in the developmental stage, which need to be further validated and improved in clinic practice. Targeted, feasible, and effective oral care management programs should be developed and implemented according to the specific patient and hospital circumstances.

Objective: To integrate the qualitative research about the real experience with ileal conduit after radical cystectomy among patients with bladder cancer, fully understand the patient's problems and needs, and provide implications for formulating specific intervention strategies of urostomy patients. Methods: The qualitative studies on real experience for ileal conduit after radical cystectomy among patients with bladder cancer were systematically searched, evaluated, and extracted from the Chinese and English databases according to the inclusion criteria. The retrieval time limit is from the inception to March 2023. The literature quality was evaluated by the Quality Evaluation Standard for Qualitative Research of Evidence Based Health Care Center of Joanna Briggs Institute in Australia (2020), and the Meta integration method was used to integrate the results. Results: A total of 12 studies, consisting of 61 research results were included. A total of 10 new categories, 4 comprehensive results were finally integrated, including Psychological problems as hard to adapt to the changes in body and lifestyle after surgery; Decreased quality of life due to changes in physiological function after surgery; Insufficient postoperative self-care ability and eagerness of professional continuing care guidance; Social support for life reconstruction from multiple resources as patient has post-traumatic growth. Conclusion: To improve the quality of life of patients after operation, the health care professionals should fully understand the postoperative experience and needs of patients promote the medical security system, and provide patients with long-term, comprehensive professional guidance and continuous care services.

Objective: To construct the feedback model of on-site nursing quality supervision and verify its effect, in order to provide guidance for clinical nursing quality supervision. Methods: Based on the SAID feedback communication model, literature analysis, semi-structured interview and questionnaire survey were used to clarify the attitudes, feelings and needs of supervisors and supervised on on-site supervision of nursing quality, and thematic group discussion was used to form the item pool of feedback mode on on-site supervision of nursing quality. Twenty-one experts were selected nationwide and two rounds of correspondence consultation were conducted by Delphi method. The feedback model was formed and applied in the two hospitals, and the evaluation of supervisors and supervised on on-site supervision of nursing quality before and after the application of this model was compared. Results: The feedback model of on-site supervision of nursing quality was formed, including 4 links and 16 requirements, and the 4 links were: identifying the problem, stating the situation, illustrating the impact, and direct improvement. The effective questionnaire recovery rates were 91.7% and 95.5%, respectively. The degree of expert authority was 0.90 and 0.92, respectively. Kendall harmony coefficients of experts were 0.17 and 0.24 (P<0.05), respectively. After the application of the model, the score of on-site nursing quality supervision of the supervisors and the supervised were higher than those before (P<0.01). Conclusion: The formation process of the feedback model of on-site nursing quality supervision is scientific and standardized, and it can be used as a reference for nursing managers to feedback problems in on-site supervision of nursing quality.

Objective: To investigate the clinical efficacy of the electro-heating bian-stone warming and ironing technique in elderly patients with functional constipation due to spleen-kidney yang deficiency. Methods: From July 2023 to February 2024, 58 patients with functional constipation due to spleen-kidney yang deficiency who were admitted to a tertiary grade A Traditional Chinese Medicine hospital in Beijing were recruited as the study participants. They were randomized into either an experimental group (n=29) or a control group (n=29). Patients in the control group received abdominal massages on the basis of health guidance. On the other hand, patients in the experimental group received electro-heating bian-stone warming and ironing therapy on the basis of health guidance. The improvement in patients' constipation was evaluated using the Patient Assessment of Constipation Symptom (PAC-SYM), Bristol Stool Form Scale (BSFS), Patient Assessment of Constipation Quality of Life (PAC-QOL), and records of weekly defecation frequency before and after the intervention. Results: The PAC-SYM, BSFS, and PAC-QOL scores for both the experimental and control groups were compared before and after the intervention, showing statistically significant differences (P<0.05). After the intervention, there were notable differences in PAC-SYM, BSFS, PAC-QOL, the weekly defecation frequency significantly between the experimental and control groups (P<0.01). Conclusion: The electro-heating bian-stone warming and ironing technique can significantly improve functional constipation symptoms in elderly patients with functional constipation due to spleen-kidney yang deficiency, increase defecation frequency, and enhance the patient's quality of life.

Objective: To explore the effects of interventions based on well-being PERMA model on subjective well-being of parents of children and adolescents with type 1 diabetes and their cooperation and support to children. Methods: A convenient sampling method was used to select two units in a tertiary Grade A hospital in Gansu Province. Eighty T1DM children or adolescents and their parents who met the inclusion criteria were selected and then assigned to the control group (40 cases) and the experimental group (40 cases). Parents in the control group were given routine care, while the parents in the experimental group were given a well-being PERMA nursing intervention besides routine care. The score of the Collaborative Parent Involvement scale (CPI) of patients and Index of Well-being (IWB) of parents were evaluated before and after the intervention. Results: Before the intervention, there were no statistically significant differences between the two groups in general information, the score of Collaborative Parent Involvement scale and IWB. After intervention, the score of CPI and IWB in the experimental group were statistically higher than those in the control group (P<0.05). Conclusion: Well-being PERMA nursing interventions on parents can improve the CPI and IWB of parents with children of T1DM.

Objective: To evaluate the application effects of "Tree of Life" collective narrative practice in family caregivers of hospice care and provide practical basis for the care work development of caregivers. Methods: A total of 76 family caregivers of advanced cancer patients receiving hospice care at a tertiary Grade A tumor specialist hospital in Urumqi from January 2023 to June 2023 were randomly divided into control group (n=37) and experimental group (n=39). The experimental group adopted the collective narrative practice of "Tree of Life" on the basis of the control group, while the control group adopted routine admission education and psychological nursing. The Anticipated Grief Scale (AGS), Herth Hope Index (HHI) and Quality of Life Scale for Family Caregivers with Life-threatening Diseases (QOLLTI-F) were used to evaluate the intervention effects before and after intervention. Results: After intervention, the scores of HHI and QOLLTI-F in the experimental group were higher than those in the control group, and the scores of AGS were lower than those in the control group, with statistically significant differences between the two groups (P<0.05). Conclusion: The collective narrative practice of "Tree of Life" can effectively reduce the expected sadness of caregivers, which is conducive to increasing the level of hope and quality of life of family caregivers.

Objective: To develop a hospice hierarchical referral service system and to evaluate its effects with the expectation of support provision for the development of hospice referral mechanisms at a later stage. Methods: The hospice hierarchical referral service system was determined by literature review, thematic analysis, Delphi expert inquire method. A quasi-experimental study design was adopted and 80 patients were randomly recruited from the medical institutions which were involved in the hierarchical referral service system in Nanjing. The upward and downward referral rate of the patients, satisfaction of patient's family members on the hierachical referral service, and the quality of death were collected and compared. Results: The positivity coefficients of two rounds of expert inquiries were both 100.0%. The authority coefficient of experts was 0.800 and 0.825, and the coordination coefficients were 0.141 and 0.632, respectively (P<0.01). The finalized hospice hierarchical referral service system included 4 first-level indicators (standards for hospice care in all levels of healthcare institutions, referral standards of patients, information-sharing mechanism, and quality control system), 14 second-level indicators, and 70 third-level indicators. The upward and downward referral rate, and quality of death were all optimized. Conclusion: The hospice hierarchical referral service system is scientific, reliable, specialized, and practical, which can provide important reference for better implementation of hospice hierarchical referral service.

Objective: To evaluate the reliability and validity of the Chinese version of the Pediatric Quality of Life Inventory Infant Scales (PedsQLTM Infant Scales), and to provide a tool for the assessment of infant quality of life in China. Methods: A total of 578 infants aged 1-12 months who received outpatient pediatric healthcare services from December 2023 to January 2024 in 29 community health service centers in Shanghai were selected by multi-stage probability sampling method. The caregivers of infants were surveyed using the Chinese version of the PedsQLTM Infant Scales. Results: The correlation coefficients between the scores of each item and the total score of the Chinese version of the PedsQLTM Infant Scales ranged from 0.544 to 0.799 (P<0.05). There were statistically significant differences in the critical values of each item between high and low groups (P<0.05). The overall Cronbach’s α coefficient for the scale was 0.967, and the split-half reliability coefficient was 0.888. The cumulative variance of exploratory factor analysis was 71.017%. Confirmatory factor analysis result demonstrated a good fit for the model. Conclusion: The Chinese version of the PedsQLTM Infant Scales for infants aged 1-12 months exhibits good reliability and validity, and it provides a scientifically reliable tool for the evaluation of infant quality of life in China.

Objective: To develop a Digital Comprehensive Geriatric Assessment (DCGA) system, and explore its application effect in elderly health assessments within nursing homes, providing scientific support for personalized health interventions. Methods: The DCGA system was developed based on the national standard "Geriatric Assessment Guidelines" and international Comprehensive Geriatric Assessment framework. Selecting elderly individuals residing in a nursing home in Hangzhou between July and October 2024 as the study subjects using convenience sampling method. The DCGA system was used to assess self care ability, cognitive function, and frailty state. The efficiency and consistency of system evaluation and manual evaluation were compared. Results: The DCGA system had four core functions: full-process information management (elder list module); dynamic updated elderly profiles (elder profile module); multi-dimensional health assessment (elder assessment module); intelligent reports and decision support (elder report module). The system successfully completed health assessments for 196 elderly individuals. The assessments revealed that 95.9% of the elderly individuals had varying degrees of functional dependency, 75.0% exhibited cognitive impairment, 57.6% were in a state of frailty or pre-frailty. The average time for using the DCGA system and manual evaluation was 12 minutes and 45 minutes, respectively. The two evaluation methods had a high degree of consistency in the determination of ability level classification results (Kappa coefficient of 0.86). Conclusion: The DCGA system enables a comprehensive assessment of elderly health. Its standardized assessment process and data visualization features provide effective support for health management in nursing homes, demonstrating strong potential for wider application.
