Development of implementation strategies for perineal warm compresses in the second stage of labour based on the CFIR framework

Abstract: Objective: To develop clinical implementation strategies for perineal warm compresses in the second stage of labour, in order to promote their routine adoption and dissemination in clinical practice. Methods: Guided by the Consolidated Framework for Implementation Research (CFIR), a mixed-methods study was conducted to identify barriers and facilitators to the application of this technique. A modified nominal group technique was employed to determine the key barriers. The Consolidated Framework for Implementation Research-Expert Recommendations for Implementing Change matching tool (CFIR-ERIC) was utilised to generate strategies corresponding to these key barriers, which were subsequently refined through a Delphi expert consultation. Results: The study identified 12 barriers to the implementation of the technique. Following prioritisation, seven key barriers were selected, including the high complexity of the technical procedure, lack of standardised operational protocols, and the absence of practice habits among healthcare professionals, etc. A bundle of five implementation strategies was developed: optimising warm compresses equipment and materials, establishing standardised operating procedures, developing and distributing educational materials, conducting training and cultivating key trainers, and forming a collaborative group for dynamic evaluation. Conclusion: The clinical implementation strategy for perineal warm compress during the second stage of labor developed in this study is highly targeted, comprehensive, and of high scientific validity and clinical reference value.
