Study on the element combinations of the community- and home-based integrated care service models in megacities

Abstract: Objective: To analyze the combinations of service elements within community-and home-based integrated care service models across the urban-suburban-rural areas of megacities, and to provide theoretical basis for optimizing regionally adaptive services. Methods: Guided by the Complex Integration Theory framework, this study adopted a descriptive qualitative research approach and employed purposive sampling from July to December 2024 to select seven communities with experience in integrated care services in urban, suburban, and rural areas of Beijing. Data were collected through thematic roundtable discussions and retrieval of relevant news reports. Text analysis was applied to analyze the data. Results: The study identified sixteen key service elements, forming five integration dimensions: structural integration centered on cross-agency collaboration and diversified payment systems, functional integration characterized by basic support and flexible service expansion, normative integration guided by policy advocacy and value principles, interpersonal integration supported by multidisciplinary teams and neighborhood mutual assistance, and process integration leveraging digital connectivity and the incorporation of traditional Chinese medicine services. Conclusion: The community- and home-based integrated care service models across the urban-suburban-rural areas in the megacity exhibited both shared foundational features and distinct patterns in the combination of elements due to differences in regional resource characteristics and population profiles. Efforts should be made to advocate for a flexible pathway characterized by a "standardized framework with regional adaptation" for the development of community- and home-based integrated care services. This pathway will enable optimal allocation of service resources through precise alignment with urban-rural characteristics, and promote both equitable access to and efficient integration of community- and home-based care services.
