Abstract: Objective: To summarize the home-based care experiences for patients with disability, and explore working mode of the home care program. Methods: The authors provided home care for 15 patients with disability and analyzed the effects. Results: The care ability of family caregiver improved, and their mental pressure decreased obviously. All of them were satisfied with this service. Conclusion: Patients' travel back and forth hospital can be effectively avoided through home care.Moreover, patients' self care ability increased, and the occurrence and development of complications decreased after home care.
