Abstract: Objective: To explore the effect of authorization education mode of "Hospital-community-family" linkage in patients with insulin pump therapy. Methods: Sixty patients with diabetes using the insulin pump therapy from 2012 February to 2014 March in Department of Endocrinology received the "hospital-community-family" trinity authorized education mode,and then the changes of fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin (HbAlc) level and selfcare ability were measured at 3 and 6 months after follow-up. Results: The implementation of authorized education after 3 and 6 months, the fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin (HbAlc) level and self-care ability were significantly improved compared with before, the differences were statistically significant (P<0.05). Conclusion: The authorized education mode of "hospital-community-family" linkage integrates the health resource of hospital and the community,and gives full play to synergy of family members. This mode can improve the self-care ability of patients with diabetes, increase adherence to treatment of patients, and improve the clinical metabolic parameters in patients with diabetes mellitus.
