Assessment of moderate and severe dementia: the perspective of dementia care

Abstract: With the global cognitive deterioration in moderate and severe dementia, persons with dementia present more mood problems and challenging behaviors and rely on care by others. Assessment lays the foundation for developing individualized dementia care plan. The review first used the IDEAL schedule for staging care of dementia as an example to illustrate how to conduct integrative assessment from the perspective of person-centered dementia care. Second, the paper reviewed the components and major approaches of assessment for individualized care for moderate and severe dementia. Then, a case study demonstrated how to make an assessment-based individualized care plan. Lastly, the gap in research and implementation of the integrative assessment in the dementia care practice was discussed. The review also shared perspectives on the future priorities of research and practice on care for moderate and severe dementia.
