Meta-analysis of the incidence and prevalence of Medical Device-Related Pressure Injuries in adult patients

Abstract: Objective: To explore the incidence and prevalence of Medical Device-Related Pressure Injuries(MDRPI) in hospitalized adult patients by meta-analysis. Methods: A comprehensive electronic literature search of PubMed, Embase, The Cochrane Library, CINAHL, Web of Science, CNKI, Wanfang Data, VIP and SinoMed databases was conducted from inception to 31st January, 2019. Studies that reported the prevalence or incidence of MDRPI were included in the review. Results: A total of 21 articles were included in the meta-analysis. The total sample size was 193,456, and 1,406 patients were developed MDRPI. About 9 articles reported the incidence of MDRPI in hospitalized adult patients, and 11 articles reported the prevalence of MDRPI in hospitalized adult patients, 4 articles reported the prevalence of MDR-HAPI in hospitalized adult patients. Meta-analysis showed that the incidence of MDRPI in adult hospitalized patients was 10.3%[95%CI(6.4%-14.2%)], and the prevalence was 6.1% [95%CI(5.0%-7.1%)]. The prevalence of MDR-HAPI was 1.1% [95% CI (0.5%~1.7%)]. Subgroup analysis showed that prospective studies (16%) have a higher incidence of MDRPI than that in retrospective studies (4%); The prevalence of MDRPI in adult patients increased in 5 recent years; the incidence and the prevalence of ICU and trauma center were the highest in all the departments; MDRPI is concentrated in stage 1, stage 2 and mucosal injury, with head, face and neck being common sites. MDRPI is associated with the use of respiratory support and preventive devices. Conclusion: The incidence of MDRPI in adult patients is high. Among them, ICU and trauma center patients are in high risk and need to be paid more attention. It is more important in clinical setting to prevent Stage 1, Stage 2 and mucosal injury. Head, face and neck area are common sites for MDRPI. It is necessary to check this related compression areas of respiratory support and preventive devices. We should investigate the MDRPI prevalence every year to obtain the current status of MDRPI and provide a reference for clinical nursing work.
