Abstract: Objective: To develop a home monitoring and follow-up system for long-term diet management, in order to improve chronic kidney disease dietary self-management. Methods: 30 patients with stage 3 chronic kidney disease were enrolled. Target protein intake was set according to patient's laboratory indicators and general condition. The Dietary Intake and Monitoring System APP installed on patient's mobile phone collects daily dietary intake. Patients received home e-feedbacks and transmitted data to nurses who adjusted diet management accordingly. 24-hours urine was collected before and 2 months after the intervention, and the daily sodium and DPI mean values were calculated. Results: Daily sodium and DPI intakes were significantly improved (P<0.05) after intervention. Conclusion: The home monitoring and follow-up system on chronic kidney disease can effectively improve patients' self-management ability in protein and sodium intake, and positively promote disease rehabilitation.
