APACHE II和改良NAS在神经内科ICU护理人力资源配置中的适用性研究

The applicability of APACHE  II and modified NAS scores among hospitalized patients in predicting nurse staffing allocation in the neurology ICU

摘要: 目的:研究急性生理与慢性健康状况评分II(Acute Physiology and Chronic Health Evaluation II,APACHE II)、改良版护理活动评分(Nursing Activities Score,NAS)在预测成人神经内科ICU护理人力需求配置中的适用性。方法:分别采用APACHE II、改良NAS对神经内科ICU?314例次的患者进行评分,预测护理人力的配置需求,并与实际护患比进行比较。结果:APACHE II预测护患比明显高于改良NAS和实际护患比,差异有统计学意义(P<0.05),改良NAS预测和实际护患比差异无统计学意义(P>0.05),APACHE II预测护患比最高(0.50∶1.00~1.50∶1.00);APACHE II在不同GCS评分、不同住院时长以及不同疾病诊断中预测的护患比需求最高,改良NAS在不同班次中预测护患比差异有统计学意义(P<0.05)。结论:神经内科ICU患者病情危重、住院时间较长同时伴有并发症时, 可采用APACHE II对人力进行调整;不同班次尤其白班且基础护理工作量较大时,可采用改良NAS进行人力资源配置。

Abstract: Objective: To explore the applicability of the Acute Physiology And Chronic Health Evaluation II (APACHE II) and modified the Nursing Activities Score (NAS) among hospitalized patients in prediction of nurse staffing allocation in the adult Neurology Intensive Care Units (ICU). Methods: Totally 314 cases in the Neurology ICU were estimated using APACHE ?II and modified NAS to predict the demands for nurse staffing. The predicted nurse staffing were compared with the actual nurse-patient ratio in the unit. Results: Significant differences were found between the actual and predicted nurse-patient ratio according to patients' scores on APACHE ?II and modified NAS. The actual nurse-patient ratio remained was the lowest, the modified NAS prediction was in the middle, while the APACHE ?II prediction was identified as the highest. The APACHE??II prediction of nurse-patient ratio ranged from 0.50?:?1.00 to 1.50?:?1.00; APACHE ?II was more applicable to predict nurse-patient ratio in nurse staffing regard to the care of hospitalized patients with different Glasgow Coma Scale (GCS), different length of hospital stay and different disease diagnosis. Significant differences were found between day sfift and night shift according to modified NAS. Conclusion: APACHE??II can be used to adjust the manpower demand of ICU patients in Neurology ICU when patients are serious, the length of hospital stay is long and there are complications; NAS can be used for human resource allocation in different shifts, especially in daytime and with large amount of basic nursing.
