O2O移动医疗教育管理模式在初始基础胰岛素治疗糖尿病患者中的应用

摘要: 目的:评价线上线下移动医疗教育管理模式在初始使用基础胰岛素治疗糖尿病患者中的应用效果。方法:采用由医生、护士和患者组成的线上线下一体化信息管理系统,对入组的初始使用基础胰岛素治疗的2型糖尿病患者进行首日教育和基线数据采集,在随后12周内,通过短信通知和电话随访追踪其空腹血糖(FPG)和基础胰岛素剂量数据,并结合线上线下的患者教育活动进行健康教育指导和血糖控制管理。结果:共纳入2型糖尿病患者1?051例,其中完成12周随访852例,随访期内患者总体FPG达标率为84.3%。广义估计方程结果显示入组后第1、2、4、8、12周患者FPG与入组前比较均下降,患者初始FPG达标率、第1、2、4、8周FPG达标率分别为第12周FPG达标率的0.009、0.098、0.172、0.260、0.286倍,未完整提交5次自我血糖监测数据的患者FPG达标率是提交完整组的0.658倍,从未参加健康教育活动的患者FPG达标率为至少参与过一次活动的患者的0.770倍,差异均有统计学意义(P<0.05)。结论:O2O移动医疗教育管理模式可提高初始使用基础胰岛素治疗患者的血糖管理水平,值得推广应用。

Abstract: Objective: To evaluate the effectiveness of the mobile medical care model based on Online-to-Offline in patients with initial basal insulin treatment. Methods: A mobile health management system based on Online-to-Offline with mobile terminals for the doctors, nurses, and patients was applied in the program. For all the recruited T2DM patients, the baseline information was collected and a first-day health education was provided. In the following 12 weeks, basal insulin doses and fasting plasma glucose (FPG) were collected online by text messages or phone call, and standardized health education and glycemic control guidance both online and offline were provided to patients. Results: Totally 1 051 patients were enrolled and 852 patients completed the 12 weeks follow-up. In total, 84.3% patients reached the FPG control target in the follow-up. The result by generalized estimation equation model suggested that the patients' FPG value was decreased after 1, 2, 4, 8 and 12 weeks, The FPG control rate of patients at the baseline, in week 1, 2, 4 and 8 was 0.009, 0.098, 0.172, 0.260, 0.286 times of that in 12 week, respectively, The FPG control rate of the patients with incomplete self-monitoring blood glucose data was 0.658 times of that of the complete group. The FPG control rate of the those who had never participated in the education program was 0.770 times of that of others (P<0.05). Conclusion: The mobile medical care model based on Online-to-Offline is an effective way to improve the blood glucose management level of patients treated with basal insulin.
