Abstract: Objective: To form the community nursing needs questionnaire for chronic disease patients, to measure its reliability and validity, and to provide scientific assessment tools for understanding and meeting the community nursing care needs of chronic disease patients. Methods: On the basis of literature analysis, theoretical research and expert consultation, we formed the questionnaire, including 5 dimensions which were health education, nursing technical support, disease management, health promotion and psycho-social support. The validity and reliability of the questionnaire were evaluated with 256 patients with chronic diseases. Results: The Cronbach's α of the total scale and each dimension were 0.93 and 0.70~0.89, respectively, which indicated good internal consistency. The CVI of the total scale and the dimensions were 0.98 and 0.95~1.00, respectively. Five factors were extracted by exploratory factors analysis and 63.13% of total variance were explained. Conclusion: The community care needs questionnaire for chronic diseases patients has a good reliability, and its dimension setting is suitable for evaluating community care needs of patients with chronic diseases.
