维持性血液透析患者症状困扰与睡眠质量和生活质量的相关性研究

Correlation between symptom burden and sleep quality, quality of life in Maintenance Hemodialysis patients

摘要: 目的:调查维持性血液透析患者的症状困扰水平,分析症状困扰与睡眠质量和生活质量之间的相关性。方法:采用患者一般资料问卷、透析患者症状困扰量表(DSI)、中文版匹兹堡睡眠质量指数量表(PSQI) 和健康状况问卷(SF-36) 对2013 年5 月至2014 年5 月于中南大学湘雅医院和湘雅三医院血液透析中心进行维持性血液透析治疗的143 例患者进行调查。结果:维持性血液透析患者症状困扰总分为(76.69±37.95)分,经历症状数量为(13.12±5.27)项。PSQI 总分为(10.96±4.46) 分,116 例(81.11%) 患者存在睡眠障碍。且维持性血液透析患者症状困扰与睡眠质量存在正相关(r =0.527),与生活质量各维度存在负相关(r =-0.599 ~ -0.200)。结论:维持性血液透析患者存在较严重且繁多的症状困扰,睡眠质量和生活质量普遍较低,且症状困扰与睡眠质量、生活质量均存在相关性。应制定有效的症状管理措施,减轻症状困扰,提高患者的睡眠质量和生活质量。

Abstract: Objective: To investigate the level of symptom burden among Maintenance Hemodialysis patients and explore the correlation between symptom burden and sleep quality, quality of life. Methods: Totally 143 maintenance hemodialysis patients from Xiangya Hospital and the Third Xiangya Hospital of Central South University from May, 2013 to May, 2014 were recruited, general demographic data questionnaire, Dialysis Symptom Index (DSI), Pittsburgh sleep Quality Index (PSQI), and SF-36 were used to assess symptom burden, sleep quality and quality of life in maintenance hemodialysis patients. Results: The mean score of symptom burden was (76.69±37.95), the average symptom number was(13.12±5.27). The PSQI showed that 116 cases (81.11%)suffered sleep disorder, the mean score of PSQI was (10.96±4.46). Spearman correlation test showed the symptom burden was correlated positively with the sleep quality (r=0.527), and negatively with quality of life (r=-0.599~-0.200). Conclusion: Most maintenance hemodialysis patients suffered severe and many symptoms at the same time. Sleep quality and the quality of life were generally low, and symptom burden were correlated with the sleep quality and quality of life. Nurses should instruct patients to improve sleep quality and manage symptoms in order to reduce symptom burden and improve patients' quality of life.
