摘要: 新加坡预计将在2026年迈入超老龄社会。为积极应对人口老龄化带来的挑战,政府已推出多项国家层级的政策举措,如《乐龄SG》(Age Well SG)、《健康SG》(Healthier SG)、《前进SG》(Forward SG)以及《成功乐龄行动计划》(Action Plan for Successful Ageing 2023)。在医疗与社会照护体系中,整合照护始终是新加坡秉持的核心理念。世界卫生组织提出的“老年人整合照护”(Integrated Care for Older People,ICOPE)框架被引入新加坡,作为参考评估工具之一,旨在帮助政府、医疗与社会服务提供者、老年人及照护者更深入了解老年人内在能力的普遍状况,并制定更个性化的照护方案,支持老年人维持自主生活,实现居家安老。文章通过探讨ICOPE框架在新加坡的实施现状,重点分析其在老年照护体系中面临的挑战与发展机遇,为其他地区制定适合本土的ICOPE实施策略提供参考。

Abstract: As a super-aged society by 2026, Singapore has proactively addressed the challenges of ageing through various national policies, such as Age Well SG, Healthier SG, Forward SG, and the Action Plan for Successful Ageing 2023. Integrated care remains a core principle of the country's health and social care system in Singapore. Globally, the introduction of the WHO Integrated Care for Older People (ICOPE) framework as one of the assessment tools enables the government, healthcare and social service providers, older adults, and caregivers to better understand the prevalence of intrinsic capacity among seniors and to develop appropriate care plans. This supports continued independence and ageing in place. Through examining the current implementation status of the ICOPE framework in Singapore, this article focuses on analyzing its challenges and development opportunities within the aged care sector, aiming to help other regions develop locally appropriate ICOPE implementation strategies.
