Challenges and opportunities of implementing Integrated Care for Older People (ICOPE) in Hong Kong, China

摘要: 全球老龄化趋势下,世界卫生组织(World Health Organization,WHO)提出老年人整合照护(Integrated Care for Older People,ICOPE)模式,以维护内在能力为核心,通过跨部门协作构建“以人为中心”的照护体系。自2019年起,香港理工大学护理学院WHO社区健康服务合作中心被WHO指定承担关键角色,主导西太平洋区域内ICOPE实践与研究。从ICOPE筛查工具的可靠性测试到ICOPE模式在社区的实际实施,由在基层医疗机构中实施到在长期照护机构的初步实践,研究团队不断探索如何支持从业者及政策制定者同意和接受整合照护。文章系统介绍ICOPE模式在香港的实施路径,分析实施过程中面临的挑战及未来发展机遇,以期为其他国家和地区整合式照护提供参考依据。

Abstract: With the global trend of population ageing, the World Health Organization (WHO) has introduced the Integrated Care for Older People (ICOPE) model, which emphasizes the maintenance of intrinsic capacity as a core and advocates for the development of a person-centred care system through cross-departmental collaboration. Since 2019, Hong Kong Polytechnic University WHO Collaborating Centre (WHO CC) for Community Health Services has been designated to take a leading role in the implementation and research of the ICOPE model in the Western Pacific Region. Progressing from the investigation of the reliability of the ICOPE screening tool to the implementation of the ICOPE model in community, from the adoption of ICOPE in primary healthcare settings to the preliminary trial in long-term care settings, the Hong Kong WHO CC team continues to explore how to support practitioners and policy makers to agree and accept integrated care. This paper provides a comprehensive account of the adaptation of the ICOPE model in Hong Kong, China, critically examines the challenges encountered throughout the process, and discusses future opportunities for development, with the aim of providing evidence for other nations and regions for consideration when implementing ICOPE in the society.
